We previously developed a computerized clinical decision support system based on national consensus guidelines and previous studies. This system was used to assess the risk of venous thromboembolism. In this study, we examined the risk factors for venous thromboembolism in patients who underwent lower limb orthopedic surgery using our risk scoring system, to investigate the association between the total risk score and the occurrence of venous thromboembolism. ⋯ Our risk scoring system may be used preoperatively to determine the need for venous thromboembolism prophylaxis. This study suggests that age (≥68 years) may be a risk factor for venous thromboembolism after lower limb orthopedic surgery. Additional studies are needed to validate these results.

Uncontrolled blood pressure among hypertensive adults with rheumatoid arthritis in Saudi Arabia: A cross-sectional study.
Despite the availability and advancement of diagnostic and treatments with demonstrated benefits in minimizing cardiovascular morbidity and mortality, hypertension control rates remain suboptimal. Therefore, this research aimed to determine the prevalence of uncontrolled BP in rheumatoid arthritis (RA) patients and understand all potential risk factors for uncontrolled BP. We conducted a cross-sectional study on RA patients in 2 rheumatology clinics in 2 public hospitals in Riyadh. ⋯ Among the patients with comorbid conditions, those with obesity, hyperlipidemia, diabetes, anemia, cancer, and reflex or gastroesophageal reflux disease also showed a significantly higher risk of uncontrolled BP (P < .05). The rate of uncontrolled BP was found to be alarmingly high in the study population. Age, gender, smoking, diabetes, obesity, hyperlipidemia, cancer, gastroesophageal reflux disease, and osteoporosis are independently linked with lack of BP control.

The present study aimed to investigate the comprehensive expression profiles of long non-coding RNA (lncRNA) in ankylosing spondylitis (AS). The peripheral blood samples were collected from 6 AS patients and 6 age- and gender-matched healthy controls (HCs), and separated for peripheral blood mononuclear cells, followed by RNA-sequencing. ⋯ Principal component analysis plots indicated that both lncRNA and mRNA expression profiles could distinguish AS patients from HCs; heatmap diagram exhibited a relatively good consistency and tendency of lncRNA and mRNA expression profiles in AS patients and HCs, respectively; volcano plots exhibited 114 upregulated and 45 downregulated DElncRNAs, 284 upregulated and 435 downregulated DEmRNAs in AS patients compared with HCs; Gene ontology enrichment analyses indicated that DElncRNAs (based on cis-target and trans-target genes) and DEmRNAs were enriched in molecular functions (including DNA binding, protein binding, etc) and biological process (including immune response, inflammatory response, etc); Kyoto Encyclopedia of Genes and Genomes enrichment analyses revealed that these DElncRNAs (based on cis-target and trans-target genes) and DEmRNAs were enriched in immune and inflammation-related signaling, such as B cell receptor signaling pathway, TNF signaling pathway, NF-kappa B signaling pathway, etc. Our study displays the comprehensive expression profiles and functions of lncRNAs involved in AS, which provides reference for further researches discovering candidate lncRNAs with value in assisting early AS diagnosis.

Effect of pursed-lip breathing and forward trunk lean positions on regional chest wall volume and ventilatory pattern in older adults: An observational study.
Pursed-lip breathing (PLB) and forward trunk lean posture (FTLP) are commonly used to relieve dyspnea and improve ventilation in a rehabilitation program. However, their effect on chest wall volumes and movements in older adults without chronic obstructive pulmonary disease has never been investigated. This observational study aimed to identify the effect of combined PLB and FTLP on total and regional chest wall volumes, ventilatory pattern, and thoracoabdominal movement using in older adults. ⋯ The ML diameter changes at the umbilical level were significantly greater during the PLB + FTLP than the QB + UP (P < .05). However, no significant difference in the relative regional chest wall volumes and phase angle among the experimental tasks (P > .05). In conclusion, a combined PLB performed in an FTLP or UP sitting could be used as a strategy to improve chest wall volumes and ventilation in older adults.

Breast cancer (BC) is the most common malignant cancer in women. A predictive model is required to predict the 5-year survival in patients with BC (5YSPBC) and improve the treatment quality by increasing their survival rate. However, no reports in literature about apps developed and designed in medical practice to classify the 5YSPBC. This study aimed to build a model to develop an app for an automatically accurate classification of the 5YSPBC. ⋯ The 15-variable CNN model with 38 parameters estimated using CNN for improving the ACC of the 5YSPBC has been particularly demonstrated in Microsoft Excel. An app developed for helping clinicians assess the 5YSPBC in clinical settings is required for application in the future.
