Case ReportsSudden onset sensorineural hearing loss caused by meningeal carcinomatosis secondary to occult malignancy: report of two cases.
Meningeal carcinomatosis (MC) is an uncommon form of metastasis of solid tumors. In the absence of clinical meningeal or parenchymal involvements, the sensorineural hearing loss (SNHL) as the starting symptom of MC is very infrequent. We report the history of two patients affected by MC who presented first with progressive SNHL. ⋯ Only the histopathologic result and surgical biopsy in cases 1 and 2, respectively, identified masses secondary to occult malignancy. However the available investigations could not discover the primary site of metastatic carcinoma. Despite the poor prognosis, because of the rarity and severity of MC we consider important to make known our experience in order to consider metastatic tumors in the differential diagnosis for sudden SNHL.

Vestibular function outcomes after vestibular neurectomy in Meniere disease: can vestibular neurectomy provide complete vestibular deafferentation?
Vestibular neurectomy is considered the reference treatment of incapacitating vertigo accompanying Meniere disease, with an efficiency rate of 85-95% in most literature reports. The aim of this study is to evaluate if vestibular neurectomy can provide a complete vestibular deafferentation by investigating complete vestibular function after surgery. ⋯ This study proves that vestibular neurectomy can provide a complete vestibular deafferentation. We discuss this vestibular evaluation protocol and the main difficulties encounter during surgery, which could lead to partial nerve section and partial relief, and explain residual vestibular function after vestibular neurectomy.

Case ReportsUpper airway obstruction by epiglottis and arytenoids hematoma in a patient treated with warfarin sodium.
With the increase in the number of patients undergoing warfarin therapy, reports of complications due to such therapy have become frequent. Although upper airway obstruction secondary to bleeding resulting from warfarin therapy is rare, it is a life-threatening complication because of the risk of airway obstruction. Only one previous case of hematoma of the epiglottis and arytenoids has been reported. ⋯ The left true vocal cord was erythematous, but the airway was adequately maintained. The PT-INR of the patient was 10. She was managed conservatively and had a good course.

Case ReportsMiddle turbinate osteoma extending into anterior cranial fossa.
Middle turbinate osteoma is very rare and only two times have been reported before. We reported a 31-year old male presenting middle turbinate osteoma that extending into the anterior cranial fossa, causing pneumocephalus. The osteoma was resected by combining endoscopic sinus surgery with bifrontal craniotomy and the patient was relieved of headaches.

Case ReportsPartial maxillary swing approach for removal of the tumors in the retromaxillary area.
Removal of tumors around the pterygoid process poses difficult challenges. Our method for removal of tumors in this area using a modification of the maxillary swing approach is reported. ⋯ The partial maxillary swing approach offers a wide surgical field of the lesion around the posterior part of the maxilla and pterygoid process without severe complications and is useful for removal of tumors in these areas.
