The aim of this prospective, single-blind study was to assess the variations in the blood levels of PaO(2), PaO(2)/FiO(2), PaCO(2), and acid-base balance of patients undergoing laryngeal microsurgery under general anesthesia using small-bore endotracheal tubes. ⋯ Laryngeal microsurgery under general anesthesia can be performed using small-bore endotracheal tubes. This is not likely to have any adverse effects on a patient's blood gases and acid-base balance unless the operation lasts longer than 105 min.

Case ReportsPost-traumatic pseudomeningocele presenting as a cyst of external auditory canal: report of a case.
Pseudomeningoceles are formed by extravasation of cerebrospinal fluid through a dural defect into soft tissue. They mostly form as a result of surgical trauma to the dural covering of the lumbar or cervical spine surgery, especially during laminectomy. Howerver, post-traumatic pseudomeningocele rarely occurs in the head and neck. ⋯ The right ear was explored and mastoid antrum was partially filled with a cyst connected to the dural defect. The extradural portion of the mass was removed, the dural defect was repaired with a temporalis fascia-cartilage graft. Clinical manifestations, diagnosis and surgical approaches for post-traumatic pseudomeningocele arising in the head and neck region are briefly discussed.

Case ReportsA newborn case of congenital laryngeal cyst complicated with pneumothorax and pneumomediastinum.
Benign congenital laryngeal cysts are rare entities. They often cause chronic hoarseness and severe stridor. Case reports of congenital laryngeal cyst complicated with pneumothorax and pneumomediastinum are very rare. ⋯ The stridor had disappeared and the pneumothorax and pneumomediastinum were improved without further medical intervention. The histopathological examination revealed that the cystic wall consisted of normal squamous epithelial cells. It is reasonable to think that the high airway pressure due to congenital laryngeal cyst was responsible for pneumothorax and pneumomediastinum.

Case ReportsRetrofacial approach of cochlear implantation in inner ear malformation with aberrant facial nerve: a case report.
Cochlear implantation is regarded as a safe surgery for young children with minimal complications. However, inner ear malformations and aberrant course of facial nerves may impede electrode insertion via the round window approach and increase the risk of iatrogenic facial nerve injury. We report a case of cochlear incomplete partition in a patient with anomalous facial nerve anatomy. ⋯ A retrofacial approach was used to expose the round window and the electrode was inserted successfully. No surgical complications were found postoperatively, and the child showed significant improvement in speech perception. As the course of the aberrant facial nerve is difficult to track preoperatively, surgeons should proceed with caution to reduce the risk of facial nerve injury during the operation.

Case ReportsMalignant myoepithelioma arising from recurrent pleomorphic adenoma of the soft palate.
Malignant myoepithelioma is a rare salivary gland neoplasm that can arise either de novo or within a pre-existing pleomorphic adenoma. We report a case of malignant myoepithelioma arising from a pleomorphic adenoma of the soft palate. A 70-year-old woman presented in our department with a very large tumor of the soft palate. ⋯ Histological examination of the recurrence revealed a malignant spindle cell neoplasm with an infiltrative growth pattern and a high mitotic rate. Immunohistochemical investigation confirmed the diagnosis of a low-grade carcinoma of myoepithelial origin with free surgical margins. The patient remains free of disease for more than twelve months after the end of treatment.
