Pars plana vitrectomy results of exogenous endophthalmitis in children.
To identify the etiology, culture results, pars plana vitrectomy results, complications, and visual outcome of exogenous endophthalmitis in children. ⋯ Early pars plana vitrectomy and intravitreal vancomycin plus ceftazidime injection improved anatomic and functional success in pediatric eyes with exogenous endophthalmitis.

Periorbital necrotizing fasciitis: outcomes using a CT-guided surgical debridement approach.
The surgical management of necrotizing fasciitis usually involves early radical/wide and aggressive debridement of involved areas. We describe 5 cases of periorbital necrotizing fasciitis (NF), managed using a computed tomographic (CT)-guided approach to surgical debridement. ⋯ Periorbital NF behaves differently from NF of other areas. CT-guided surgical debridement of the superficial muscle maximizes preservation of healthy tissue and facilitates reconstruction. Delayed reconstruction allows fibrosis to settle and good cosmetic and functional results are possible. However, NF remains potentially lethal and close observation and a flexible management plan are required.

We performed this retrospective study to evaluate and report the reasons for cancellation of elective cataract surgeries in our tertiary care hospital, Kuwait University Hospital, from January 2001 until December 2003. Proportion of elective cataract surgeries canceled on the day of surgery was obtained from the operating theater list and reasons for cancellations were recorded. ⋯ Postponing elective cataract surgery for patients scheduled for surgery is a stressful situation for the patients and their relatives. Most reasons (80%) for cancelling the surgery were avoidable and efforts to reduce the number of postponed cases should be increased. Medical fitness should be done properly to avoid unexpected rise in blood pressure and blood sugar on the day of surgery.

To determine whether topical tranexamic acid can provide acceptable therapeutic concentration in the aqueous humor. ⋯ Topical administration of tranexamic acid is effective in yielding therapeutic intraocular concentrations of drug without any ocular or systemic toxicity.

Randomized Controlled Trial Comparative Study
A comparison of retrobulbar block, sub-Tenon block, and topical anesthesia during cataract surgery.
This randomized, double-blinded, prospective study was performed to compare the intraoperative hemodynamic variables and the patient-reported outcomes, such as intra- and postoperative analgesia and patient satisfaction, of retrobulbar block, sub-Tenon block, and topical anesthesia during cataract surgery under monitored anesthesia care. ⋯ Sub-Tenon block seems to be better than retrobulbar block and topical anesthesia in patient satisfaction though adequate analgesia was achieved after retrobulbar block during cataract surgery under monitored anesthesia care.
