Exercise duration rather than peak oxygen uptake better correlates with Fev1 and inspiratory capacity in chronic obstructive pulmonary disease.
Exercise duration of constant-load endurance tests has been recently demonstrated to be more sensitive in detecting the changes after bronchodilator administration than either walking distance or peak oxygen uptake in patients with COPD. In the present study, we questioned whether exercise duration of progressive load is better correlated with forced expiratory volume in 1 sec and inspiratory capacity than other indices of submaximal exercise test during stable period in patients with COPD. ⋯ There is an impairment of aerobic capacity in stable period COPD patients compared to healthy controls. Exercise in COPD patients is ventilation-limited and exercise duration with respect to peak VO(2) is better correlated with FEV(1) and IC in stable COPD patients undergoing progressive-load cycling exercise.

Extremely prolonged INR associated with warfarin in combination with both trazodone and omega-3 fatty acids.
We report the case of a 65-year-old Caucasian male who had been taking warfarin for 6 months after an episode of postoperative pulmonary thromboembolism (PTE). His medical history was otherwise insignificant. He received a prescription of trazodone and fish oil for not-very-well defined complaints. ⋯ Trazodone also has interactions with warfarin through not well-understood mechanisms. Although drug interaction reference texts classify warfarin-trazodone interaction as late-onset and clinically insignificant, this has been questioned in other studies. This particular case illustrates a possible interaction between warfarin and these two medications.

Vanadyl sulfate (VS) and taurine are two promising agents in the treatment of diabetes related to their antihyperglycemic, antihyperlipidemic, and hyperinsulinemic effects. Data about the effects of VS on the oxidant-antioxidant system is limited and controversial. However, taurine is a well-documented antioxidant agent and our aim was to investigate the effects of VS, taurine and VS and taurine combination on the oxidative-antioxidative systems in streptozotocin-nicotinamide (STZ-NA) diabetic rats. ⋯ The findings of the present study suggest that VS and taurine exert beneficial effects on the blood glucose and lipid levels in STZ-NA diabetic rats. However, VS might exert prooxidative or antioxidative effects in various components of the body and taurine and VS combination might be an alternative for sole VS administration.

It has recently been demonstrated that upregulation of proinflammatory cytokines plays an important role in induction of the late preconditioning in rodent models. The aim of this study was to determine if there was a late preconditioning effect caused by higher proinflammatory cytokines in unstable angina patients. ⋯ Upregulation of the proinflammatory cytokines in plasma by unstable angina induces late preconditioning effects and shifts the myocardium to a preconditioned phenotype upon exposure to impending stress.

Although inequality is often measured through three critical indicators-education, income and life expectancy-health-related differences are also essential elements for explaining levels of equality or inequality in modern societies. Investment and investigation in health also involve inequalities at the global level, and this includes insufficient North-South transfer of funds, technology and expertise in the health field, including the specific area of communicable diseases. Globally, epidemics and outbreaks in any geographic region can represent international public health emergencies, and this type of threat requires a global response. ⋯ This is necessary to guarantee a high level of security against the dissemination of communicable diseases in an ever more globalized world. In response to these needs, international health agencies have put a number of strategies into practice in order to contribute to the control of communicable diseases in poor countries. The principle strategies include: 1) implementation of mechanisms for international epidemiologic surveillance; 2) use of international law to support the control of communicable diseases; 3) international cooperation on health matters; 4) strategies to strengthen primary care services and health systems in general; 5) promotion of the transfer of resources for research and development from the North to the South.
