Impact of a nurse-run clinic on prevalence of urinary incontinence and everyday life in men undergoing radical prostatectomy.
To determine the prevalence of urinary incontinence (UI) in patients undergoing prostatectomy and to evaluate the impact of UI on the everyday life in order to select the patients eligible to enter a pelvic floor rehabilitation program. ⋯ Urinary incontinence remains prevalent as long as 12 months following catheter removal. Incontinence exerts a moderate to severe impact on daily life in 27.5% to 20.4% of respondents. In order to minimize the negative impact as much as possible, we advocate a pelvic floor muscle training program overseen by RNs.

Multicenter StudyStaging and defining characteristics of pressure ulcers using photographs by staff nurses in acute care settings.
The aim of this study was to determine if a digital photograph obtained by a staff nurse in the acute care setting could be used to determine staging and wound characteristics of a pressure ulcer when viewed by a panel of wound experts as compared to a bedside assessment by a wound expert. ⋯ The findings of this study indicate that a digital photograph alone cannot reliably convey the characteristics of a pressure ulcer. A digital photo, in combination with clinical information, may increase the accuracy of the assessment and documentation. Systems designed to provide digital photography wound measurements may improve consistency, along with the development of standardized education for examining digital photographs by wound experts.

Developing the standardized wound care documentation model: a Delphi study to improve the quality of patient care documentation.
The aim of this study was to validate a set of previously developed criteria for wound care documentation and to use the validated criteria as a framework for developing a wound care documentation model. The Skin Integrity component of the Finnish Care Classification, the Finnish Classification of Nursing Diagnosis, and the Finnish Classification of Nursing Interventions serve as the basis for the wound care documentation model. ⋯ The Delphi process was used to develop a wound care documentation model for use in an electronic record to promote systematic documentation of both wound assessment and wound care. Future research should address the utility of this documentation model for nurses with expertise in wound care and generalist nurses.

Comparative StudyAll at-risk patients are not created equal: analysis of Braden pressure ulcer risk scores to identify specific risks.
To improve identification of risk factors for pressure ulcer development and enhance targeted interventions and prevention strategies. ⋯ The total Braden Scale score is predictive of pressure ulcer development but does not assist the clinician to develop an individualized targeted prevention plan. In contrast, the use of subscale scores can enhance prevention programs and resource utilization by focusing care on the risk factors specific to the individual patient.
