Clinical judgment does not circumvent the need for diagnostic endoscopy in upper gastrointestinal hemorrhage.
To better understand the ability of physicians to predict the need for endoscopic therapy and to accurately predict specific endoscopic lesions, we performed a prospective, nonrandomized, observational cohort study in patients presenting with upper gastrointestinal hemorrhage (UGIH) who were undergoing endoscopy. ⋯ The accuracy of predicting the need for endoscopic therapy and the culprit cause of UGIH, based on clinical evaluation, was similar across levels of expertise. However, the faculty gastroenterologists were better than the gastroenterology fellows and the primary providers in predicting which patients do not require endoscopic treatment. We conclude that the relative inability of any group of physicians to accurately predict the presence of high-risk lesions requiring endoscopic therapy suggests that most patients with UGIH should undergo upper endoscopy for diagnosis and possible therapy.

Isoflurane is a volatile anesthetic that is widely used clinically as an inhalational anesthetic. In recent years, several studies have indicated that isoflurane has neuroprotective properties. This has led to the beneficial effects of isoflurane being analyzed in both cell culture and animal models, including various models of brain injury. ⋯ Currently, there are no well-established effective therapies for neonatal hypoxia ischemia. In line with this, isoflurane, which displays neuroprotective properties in several paradigms and has been shown to improve neurological deficits caused by brain injuries, has the capability to be an extremely relevant clinical therapy for the resolution of deficits concomitant with neonatal hypoxic ischemic brain injuries. This review therefore seeks to explore and analyze the current information on isoflurane, looking at general isoflurane anesthetic properties, and the protection it confers in different animal models, focusing particularly on neuroprotection as shown in studies with neonatal hypoxic ischemic brain injury.

Chronic hepatitis B virus (HBV) infection and liver cancer mortality represent a neglected health disparity among Asian Americans. The purposes of this study were to compare the prevalence of hepatitis B among a diverse group of 1311 Ohioans in Franklin County, OH (85% Asians, 7.5% African Americans, and 6.5% whites) and to improve access to care for high-risk Asian adults through advocacy and policy changes. ⋯ Outreach education may use community liaisons to improve screenings, education, and vaccination/treatment. A hepatitis free clinic was established in 2009 to provide culturally and linguistically appropriate treatment for low-income Asian Americans in Franklin County, OH.

In the present study, it was examined whether polymorphism of circadian locomotor output cycle kaput (CLOCK) gene 3111T/C was associated with susceptibility of Alzheimer disease (AD). ⋯ Among APOE e4 noncarriers, C carriers in CLOCK gene 3111T/C were associated with a high susceptibility of AD, but among APOE e4 carriers, the functional polymorphism of CLOCK gene 3111T/C was not associated with the susceptibility of AD.

Intra-articular corticosteroid injection (IACI) of betamethasone depot preparation is a popular procedure at the knee joint. Intra-articular corticosteroid injection in general could be associated with systemic effects including suppression of the hypothalamic-pituitary-adrenal axis. There are nearly no reports on the effect of IACI of betamethasone at the knee joint on the hypothalamic-pituitary-adrenal axis. ⋯ Intra-articular corticosteroid injection of 6 mg of betamethasone acetate/betamethasone sodium phosphate at the knee joint was not significantly associated with SAI at the time points tested.
