Case ReportsPregnancy after cryopreservation of donor oocytes and preimplantation genetic diagnosis of embryos in a patient with ovarian failure.
To describe the successful use of preimplantation genetic diagnosis to assess the prevalence of meiotic errors after oocyte cryopreservation in an oocyte donation cycle. ⋯ This case illustrates the feasibility of cryopreservation of donor oocytes combined with preimplantation genetic diagnosis for clinical use in those settings where there may be an increased risk of spindle-related abnormalities.

Comparative StudyEffect of Tisseel on expression of tissue plasminogen activator and plasminogen activator inhibitor-1.
To examine the effect of fibrin sealant on mRNA expression of factors regulating plasminogen activator activity in human peritoneal cells. Plasminogen activator activity is thought to play a pivotal role in degradation of the proteinaceous mass that develops after surgical procedures. Reduction of plasminogen activator activity, as occurs with tissue trauma, results in increased postoperative adhesion development. ⋯ Fibrin sealant, in the presence and absence of aprotinin, increases both t-PA and PAI-1 expression by human peritoneal cells; changes not seen with physiologic concentrations of fibrin sealant. These observations suggest that in addition to its ability to help achieve hemostasis, fibrin sealant affects the healing process by altering components of the plasminogen activator system, which may be of benefit in the reduction of postoperative adhesions.

Prevalence of thrombophilia in women with severe ovarian hyperstimulation syndrome and cost-effectiveness of screening.
To determine the prevalence of markers of thrombophilia in patients with severe ovarian hyperstimulation syndrome (OHSS) and to evaluate the cost-effectiveness of screening for factor V Leiden and prothrombin G20210A mutations in women entering an IVF program. ⋯ The prevalence of thrombophilia is not increased in women with severe OHSS. Screening for V Leiden and prothrombin G20210A mutation in an IVF general population is not cost-effective.

George Washington was the single most important figure in the founding of the United States of America. Numerous biographies of Washington have been written and his name is honored in countless ways, including as the name of the United States' capital. Washington has become so idealized in U. ⋯ Randall puts it, "He was content with Martha, but mystified why, year after year, he and Martha could produce no Washington heir." In this article, George and Martha Washington's inability to have children is discussed, and it is suggested that George was the likely source of the couple's infertility. The author also speculates as to the cause of Washington's infertility and its effect on the course of American history. Frank discussion of Washington's infertility might provide some comfort to men struggling with infertility today.

To determine the relative efficacy of ultrasound-guided embryo transfer and embryo transfer by clinical touch alone. ⋯ Ultrasound-guided embryo transfer significantly increases the chance of clinical pregnancy and significantly increases the embryo implantation rate.
