Intraorbital arteriovenous fistulas (AVFs) are rare lesions, and their treatment is challenging. We have presented a case of an intraorbital AVF treated with endovascular embolization through the surgically accessed basal vein of Rosenthal. ⋯ Direct catheterization of the translocated deep draining vein was useful to bypass the dangerous access to the fistula and could be a feasible alternative strategy for treating selected AVFs.

Vertebral cement augmentation techniques are routinely used to treat osteoporotic vertebral compression fractures (VCFs). In the current study, we used a state-level outpatient database to compare costs and postoperative outcomes between vertebroplasty and kyphoplasty. ⋯ Our analyses from a state-level database of patients undergoing vertebroplasty and kyphoplasty for osteoporotic VCFs show similar postoperative outcomes for the 2 procedures but a higher cost for kyphoplasty.

Endovascular clot retrieval (ECR) is the standard of care for acute ischemic stroke caused by large vessel occlusion. Reducing stroke symptom onset to reperfusion time is associated with improved functional outcomes. This study aims to develop a computational model to predict and identify time-related outcomes of community stroke calls within a geographic area based on variable parameters to support planning and coordination of ECR services. ⋯ This novel computational DES model can aid the optimization of delivery of a stroke service within a city, state, or country. By varying geographic, population, and other user-defined inputs, the model can be applied to any location worldwide.

The diagnosis of peripheral neurolymphomatosis (NL) is difficult and often delayed, because patients can have isolated, nonspecific nerve symptoms. Magnetic resonance imaging will usually show nonspecific findings of enlarged, contrast-enhancing nerves. We aimed to elucidate the mechanism behind an imaging finding we believe is pathognomonic of NL and likely of other hematologic diseases with peripheral nerve involvement. ⋯ We have presented a novel, unifying theory explaining the pathognomonic tumefactive appearance of NL. Our theory offers the first rational explanation for the radiological appearance of NL with peripheral nerve involvement. We believe that with earlier recognition of NL on imaging studies, patients will be able to receive an earlier diagnosis and undergo earlier treatment.

Clinical outcome of indirect decompression for a revision surgery, at the same level of a previous lumbar decompression, has not been reported. The purpose of this study was to investigate the efficacy of oblique lateral interbody fusion (OLIF) in revision surgery after decompression for degenerative lumbar spinal disease. ⋯ OLIF at the same level of a prior lumbar decompression provided a successful indirect decompressive effect, including expansion of the thecal sac, restoration of disc height, and subsequent improvement of foraminal stenosis. Specifically, this procedure can prevent incidental durotomy and nerve root injury, which may occur in conventional revision surgeries for direct posterior fusion.
