The current and future hotspots of laminoplasty focus on sagittal balance, comparison between other surgery in outcomes and complication, and axial symptoms in laminoplasty.

This narrative review explores the current landscape of functional neurosurgery in Africa, focusing on the challenges and opportunities facing the field. Drawing upon existing literature and expert insights, the review highlights the scarcity of resources, including trained personnel, infrastructure, and specialized equipment, as major hurdles impeding the widespread adoption of advanced neurosurgical techniques. ⋯ Moreover, the review underscores the significance of addressing cultural beliefs and stigmas surrounding neurological conditions to ensure equitable access to care and promote acceptance of neurosurgical interventions. By navigating these challenges and embracing emerging opportunities, functional neurosurgery in Africa holds the potential to significantly impact neurological health outcomes and enhance overall well-being.

Predictive modeling of early neurological deterioration in patients with Acute Ischemic Stroke.
Ischaemic stroke is the leading cause of death worldwide, and early neurological deterioration(END) occurs in 20%-40% of patients, which is the main cause of severe neurological deficits and disability, and even increased mortality. The occurrence of END is closely related to the poor prognosis of the patients, so it is important to identify the risk factors for the occurrence of END in patients with AIS and target intervention at an early stage factors and targeted intervention is of great significance. ⋯ Clinical practitioners should closely monitor high-frequency predictors of END in patients. However, the varying quality of current models necessitates the selection of models that balance performance with operational simplicity in clinical practice.

Normal-pressure hydrocephalus (NPH) is a condition characterized by an abnormal cerebrospinal fluid homeostasis in the brain, resulting in cognitive decline, gait disturbances, and urinary incontinence. Globally, the frequency of NPH becomes has become a major clinical concern with an increase in the elderly population. A lumboperitoneal (LP) shunt surgery is one therapeutic intervention, which diverts cerebrospinal fluid from the brain to the peritoneal cavity to mitigate NPH symptoms. However, LP shunt surgery can be precluded by lumbar spine degeneration. ⋯ While the result is promising, the efficacy of this method warrants validation by conducting larger studies. Nonetheless, combining unilateral biportal endoscopic and LP shunt surgeries could redefine treatment for elderly patients with NPH and spinal stenosis.

Midbrain cavernous malformations (MCMs) are rare and dangerous taken the important structures and tracts located in this segment of the brainstem. MCM treatment is still controversial, and surgical resection is basically indicated in cases of recurrent hemorrhage and progressive neurologic deterioration. The optimal moment to operate ruptured MCM is in the subacute stage. ⋯ His recovery was smooth but he maintained the previous oculomotor nerve palsy. We discuss important steps of the surgical approach, local neuroanatomy, and the microsurgical techniques for the resection of these challenging MCM. The goal is total resection of the MCM with the preservation of the developmental venous anomaly and the surrounding white fiber tracts.
