Observational StudyEndotracheal Intubation in Neonates: A Prospective Study of Adverse Safety Events in 162 Infants.
To determine the rate of adverse events associated with endotracheal intubation in newborns and modifiable factors contributing to these events. ⋯ Adverse events are common in the neonatal intensive care unit, occurring in 4 of 10 intubations. The odds of an adverse event doubled with increasing number of attempts and quadrupled in the emergent setting. Quality improvement efforts to address these factors are needed to improve patient safety.

Comparative StudyNot Just a Drop in the Bucket-Inversion of Oxymetazoline Nasal Decongestant Container Increases Potential for Severe Pediatric Poisoning.
Oxymetazoline is an over-the-counter nasal decongestant with potent alpha agonist properties. In overdoses as small as 1-2 mL, toxicity can be seen including bradycardia and respiratory depression. We demonstrated that inverting the container increased the volume delivered 20- to 30-fold compared with holding it upright in an in vitro model.

Intravenous Paracetamol Decreases Requirements of Morphine in Very Preterm Infants.
To determine whether intravenous paracetamol therapy is effective in pain therapy in premature infants. ⋯ The need for morphine decreased significantly after the introduction of paracetamol for the VLGA infants.

Multicenter StudyIdentifying Pediatric Severe Sepsis and Septic Shock: Accuracy of Diagnosis Codes.
To evaluate accuracy of 2 established administrative methods of identifying children with sepsis using a medical record review reference standard. ⋯ Sepsis specific International Classification of Diseases, Ninth Revision, Clinical Modification codes identify pediatric patients with severe sepsis in administrative data more accurately than a combination of codes for infection plus organ dysfunction.
