To determine the effect of vaginal dissection on the pudendal nerve. ⋯ All women had abnormal preoperative pudendal nerve function, supporting previous reports linking pudendal neuropathy with pelvic floor prolapse. Pelvic floor surgery involving vaginal dissection produces neuropathy of the pudendal nerve as measured by terminal motor latency.

To compare the efficacy, endocrine effects, and safety of Zoladex (goserelin acetate) and danazol in the treatment of premenopausal women with endometriosis in a multicenter, randomized, open study. ⋯ Zoladex is as well tolerated and as effective as danazol in the treatment of premenopausal women with endometriosis.

Randomized Controlled Trial Comparative Study Clinical TrialPreoperative skin preparation and intraoperative pelvic irrigation: impact on post-cesarean endometritis and wound infection.
To determine the impact of two skin preparation methods and two techniques of pelvic irrigation on the incidence of post-cesarean endometritis and wound infection in an indigent patient population. ⋯ Skin preparation with an antibacterial scrub in addition to standard povidone-iodine scrub and solution does not appear to play as significant a role in the reduction of post-cesarean endometritis or wound infection as does intraoperative pelvic irrigation with antibiotic solution.

To determine whether continuous intrapartum electronic fetal heart rate monitoring (EFM) is associated with decreased perinatal mortality and morbidity compared with intermittent auscultation. ⋯ In this controlled trial, intrapartum EFM, as the primary and only method of intrapartum fetal surveillance, was associated with decreased perinatal mortality due to fetal hypoxia but also with higher rates of surgical intervention for suspected fetal distress.

Case ReportsMyocardial infarction in the third trimester of pregnancy secondary to an aortic valve thrombus.
Myocardial infarction during pregnancy is rare, at an estimated one case per 10,000 women delivered. The overall mortality rate is 28% and is even higher in the second and third trimesters. We report a myocardial infarction secondary to a thrombus that developed on an artificial aortic valve in a 25-year-old woman at 26 weeks' gestation. ⋯ Pregnancy complicated by myocardial infarction due to a valve thrombosis in the third trimester can be managed with careful attention to controlling the cardiac symptoms and preventing premature delivery.
