Use of multiphasic oral contraceptives and hospitalizations of women with functional ovarian cysts in the United States.
A case-series report of seven women who developed functional ovarian cysts while taking multiphasic oral contraceptives has raised the possibility of an association between these two events. To address this question, we calculated the nationwide rates of hospitalization of women diagnosed with such cysts and examined the use of multiphasic oral contraceptives from 1979-1986. ⋯ Thus, there was no discernible association between taking these contraceptives and being hospitalized for functional ovarian cysts. Although this indirect assessment can neither confirm nor refute the postulated association, it suggests that if such an association does exist, its net public health impact has not been great.

Case ReportsEpidural analgesia in the management of symptomatic symphysis pubis diastasis.
The pain of symphysis pubis diastasis during and after pregnancy can be disabling. This report describes for the first time the successful use of epidural analgesia in relieving the pain of this disorder. ⋯ The epidural analgesia described allowed her to mobilize immediately and remain normally active for the remainder of her pregnancy. Early consultation with an anesthesiologist in severe cases of symphysis pubis diastasis is encouraged.

Case ReportsPrenatal sonographic diagnosis of hepatic hemangioendothelioma with secondary nonimmune hydrops fetalis.
A case of fetal hepatic hemangioendothelioma with secondary nonimmune hydrops fetalis is presented. The prenatal diagnosis, made at 27 weeks' gestation by real-time ultrasonography and pulsed Doppler ultrasound, was confirmed by angiography and pulsed Doppler ultrasound, was confirmed by angiography and autopsy after the infant was born 2 weeks later. To the best of our knowledge, this is the first report of prenatal diagnosis of such an entity.

Case ReportsCardiac tamponade in pregnancy during central hyperalimentation.
Cardiac tamponade from central venous catheterization occurs in less than 1% of cases. This report describes the first case of cardiac tamponade associated with total parenteral nutrition during pregnancy. ⋯ Maternal cardiopulmonary resuscitation was accomplished after pericardiocentesis, but maternal brain death and fetal death occurred. Diagnostic and therapeutic issues unique to cardiac tamponade during pregnancy are discussed.

Comparative StudyHysteroscopy with selective endometrial sampling compared with D&C for abnormal uterine bleeding: the value of a negative hysteroscopic view.
Hysteroscopic evaluation of 187 patients with abnormal uterine bleeding is reviewed. One hundred fifty-one patients had recent tissue sampling by a blind method. The hysteroscope with selected endometrial sampling was more accurate in 9.1% and less accurate in only 0.5% of patients. ⋯ Endometritis was the only condition missed by hysteroscopic viewing and selected biopsy. Those lesions missed by blind tissue sampling were uterine fibroids and endometrial polyps. Among 91 patients whose hysteroscopic view was negative, an abnormality was identified by tissue sampling in only one patient with endometritis.
