Pain management has become a very important part of postoperative care for total knee arthroplasty patients. Contemporary pain control has evolved from high-dose opioid in the past to state-of-the-art multimodal regimens. ⋯ Another method which is gaining popularity is intraarticular injection of anesthetic cocktail during surgery. Pre-op education can also help patients cope with their pain better.

Accurate children's weight estimation is necessary and important for emergency pediatric care, particularly for drug dosages, electrical therapy, or equipment sizes. Children's weight is difficult to measure in critically ill patients. Several methods were developed to estimate children's accurate weight. Most methods were developed from western countries. The methods used currently might not work with Thai children. ⋯ Family member estimation was the most accurate method in the present study. The family member could estimate the children's weight within 10% error of the measured weight about 85% of cases. In case where family member estimation was not available, the Broselow tape was the next accurate alternative method.

The tourniquet pain during 30 minutes after the application of calf tourniquet and ankle tourniquet were assessed and compared in 63 healthy volunteers, 32 males and 31 females whose ages ranged from 21-36 (average, 24) years. The visual analogue pain score assessed at 0, 1, 3, 5, 10, 20, 30 minutes of adequate tourniquet pressure application and after removal of the tourniquet at 0, 5 and 30 minutes were recorded. ⋯ It was also found a significant higher minimal tourniquet pressure required for the vascular occlusion distal to the tourniquet sites detected by a pulse oximeter in the ankle tourniquet group (mean, 310.8 +/- 40.8 mmHg) than the calf tourniquet group (mean, 272.5 +/- 36.9 mmHg, p = 0.024). The present study supports the use of ankle tourniquet to minimize tourniquet pain for foot surgery.

To determine the prevalence of dysmenorrhea, effect on daily activity, academic activities, quality of life, and knowledge of management in Siriraj medical students. ⋯ Dysmenorrhea in medical students has high prevalence and it has negative effects on daily activities, academic activities, and quality of life. Most of the subjects know that mefenamic acid and/or paracetamol can relief dysmenorrhea. Dysmenorrhea is a significant public health problem.

Migraine prophylaxis by anodal transcranial direct current stimulation, a randomized, placebo-controlled trial.
Migraine is a common headache syndrome in adult populations. Prophylaxis is necessary to improve the quality of life but some patients with migraine have contraindication or suffer from side effects of medication, and therefore, establishing non-medical, neuromodulatory approaches is necessary. Past evidence had shown that consecutive motor cortex (M1) stimulation with anodal transcranial direct current stimulation (tDCS) was effective to relieve central pain. ⋯ The present study suggests that anodal M1 tDCS may be a safe and useful clinical tool in migraine prophylaxis. The mechanism of action of anodal tDCS on neuromodulation in migraine patients needs further investigation.
