[Weaning from mechanical ventilation in patients with exacerbations of chronic respiratory insufficiency--personal experience].
Nineteen patients with exacerbation of chronic respiratory insufficiency treated with mechanical ventilation were included in the study. The mean weaning time from the respirator was 15.9 days (+/- 12.1), ranging from 2 to 49 days. Success was met in 9 patients using inspiratory pressure support (IPS), in 2 synchronized intermittent mandatory ventilation (SIMV), in 8 the simplest model (stepwise prolongation of spontaneous ventilation during continuous mechanical ventilation--CMV/SV). ⋯ The impression of exhaustion preceded the changes of gasometric parameters and fall of SaO2. The basis of effective weaning is proper selection of respirator parameters, providing almost identical gasometric values if individual patient's to those prior exacerbation of chronic respiratory insufficiency. The duration of weaning negatively correlated (0.25) with FEV1 values.

[Peak expiratory flow and certain spirometric indices in boys with bronchial asthma in the clinically silent period].
An analysis between relationship of PEF (measured with Vitalograph meter) and dynamic volumes (FVC, FEV1 and PEFII), maximal expiratory flow volumes (MEF25%, MEF50%, MEF75%) measured with Pneumoscreen by Jaeger was carried out in 9 year old boys (26 with atypic asthma and 36 healthy volunteers). Statistical significant differences were seen between PEFI (p < 0.001), FVC (p < 0.05), FEV1 (p < 0.01), and MEF25% (p < 0.02) in healthy volunteers in comparison with boys with atopic asthma in the clinically silent period.

[Pathogenic action of Erwinia herbicola. II. In vivo investigations].
The pulmonary response of guinea pigs to inhalation of antigen and endotoxin of Erwinia herbicola was investigated. Pulmonary parameters monitored in this study were breathing rate, cellular differential counts from bronchoalveolar lavage fluid, superoxide anion and interleukin-1 (IL-1) production by alveolar macrophages. Both agents caused an increase in breathing rate; an influx of neutrophils, lymphocytes, and red blood cells to the lung; and an enhancement of secretion of superoxide anions and IL-1 by pulmonary macrophages. The results indicate significant role of extrinsic bacilli Erwinia herbicola in respiratory pathology and show new mechanisms of their action.

[Pathogenic action of Erwinia herbicola. I. In vitro investigations].
The effects of antigen and endotoxin of Erwinia herbicola on physiological status of alveolar macrophages were studied in vitro. The studies comprised determination of the cytotoxicity of antigens + and their effects on superoxide anion and interleukin-1 (IL-1) generation by alveolar macrophages. ⋯ In lower concentrations (1-10 micrograms/ml) they stimulated the activity of alveolar macrophages, causing significant increase in superoxide anion(O2) and IL-1 generation. Superoxide anion and IL-1 production may be of importance in pathogenesis of organic dust-induced lung diseases and our results indicate that agents associated with these diseases induced their production.
