Case Reports Comparative StudyOcular manifestations of pituitary adenomas.
Fifty patients with pituitary adenomas were evaluated preoperatively by an ophthalmologist; of these, 68% had visual field defects, the most common (25%) a superior bitemporal depression and 16%, bitemporal hemianopsia. Almost 60% of patients with field defects had visual complaints; 38% had visual acuity in one eye of 20/60 or less. Patients with larger, nonsecreting tumors had more visual defects than patients with secreting pituitary adenomas.

Case ReportsAnal eroticism: two unusual rectal foreign bodies and their removal.
We have described two cases involving unusual rectal foreign bodies and their subsequent removal. Suggestions are included to aid in the removal of colorectal foreign bodies, as well as references to provide for the appropriate management of these cases in the emergency department. ⋯ Patient education of the dangers inherent in the insertion of objects into the colorectal area should be more prevalent. At the very least, all physicians need to be aware of anal eroticism and the necessity to take a careful medical history.

Comparative StudyAcidosis and other metabolic abnormalities associated with paint sniffing.
Acidosis (arterial pH 6.88 to 7.38) and many other metabolic abnormalities were found in eight patients observed during 16 hospitalizations for paint sniffing. In four episodes, a high anion gap was found, perhaps secondary to an accumulation of metabolites of toluene. The anion gap was normal during 12 episodes, of which seven were associated with a urine pH greater than 5.5 indicating renal tubular acidosis (RTA). ⋯ In two patients the urine pH, titratable acid, and ammonium excretion were determined before and after bicarbonate therapy. These results suggested distal but not proximal RTA. Our observations indicate that multiple metabolic abnormalities may result from solvent abuse and suggest guidelines for therapy.
