Transbronchial lung biopsy through the fiberoptic bronchoscope was done in 15 patients, and adequate pulmonary tissue was obtained in 14 (93.3%). A pathologic diagnosis was made in nine patients (64.3%), and through direct histologic examination of lung tissue in two patients certain disease entities were excluded, thus significantly influencing treatment. There was no significant hemorrhage and only one small pneumothorax occurred.

This study reports the psychologic findings in 30 patients undergoing this operation. Approximately one third developed postcardiotomy delirium, an occurrence consistent with that after other forms of open-heart surgery. Acceptance of the operation as opposed to preoperative anxiety, depression or denial, adequate comprehension of the proposed procedure, and a stable postoperative environment seem associated with less postcardiotomy delirium. Preventive preoperative therapeutic suggestions based on these findings are given.

Case ReportsFatal sepsis associated with an intrauterine device and pregnancy.
The 15th case of maternal death associated with sepsis in pregnancy in a patient using the Dalkon Shield intrauterine device is reported. Intrauterine sepsis appears to be a complication of the use of this device. ⋯ The management of sepsis during pregnancy associated with an in situ IUD often requires a combined medical and obstetrical approach. The removal of all Dalkon Shields is strongly recomended.
