To study the time course of nocturnal episodic and constant hypoxaemia during the first five nights after a major abdominal operation in patients not given supplementary oxygen. ⋯ In the light of these results which show the natural history of postoperative hypoxaemia without supplementary oxygen, and because postoperative hypoxaemia may be associated with dysfunction of organ systems after major operations, controlled studies of supplementary oxygen in the late postoperative period are warranted.

Randomized Controlled Trial Comparative Study Clinical Trial
Outcome at three to five years of primary closure of perianal and pilonidal abscess. A randomised, double-blind clinical trial with a complete three-year followup of one compared with four days' treatment with ampicillin and metronidazole.
To evaluate the long term results after primary closure of perianal abscess and pilonidal sinus. ⋯ A one day course of ampicillin and metronidazole is not associated with any more complications than a four day course. Perianal abscesses respond well to primary closure, but management of pilonidal abscesses and sinuses is more difficult.

To find out if concentrations of tumour necrosis factor a (TNF alpha) and interleukin-6 (IL-6) play a part in the pathophysiology of intra-abdominal infection, and try to identify patients who would benefit from immunotherapy against TNF alpha. ⋯ Perioperative TNF alpha and IL-6 concentrations correlated with the severity of intra-abdominal infection, so it is possible that patients who present with either septic shock or the sepsis syndrome may benefit from immunotherapy against TNF alpha.

To describe our experience with Swenson's operation for Hirschsprung's disease done during the neonatal period. ⋯ With the current high standards of anaesthesia and neonatal intensive care, and an experienced surgeon, Swenson's operation for neonatal Hirschsprung's disease is safe and the procedure of choice for this condition.
