Patellar problem is an important cause of nonseptic failure of the current condylar design of total knee arthroplasties. Patellar dislocation after total knee arthroplasty is infrequent but can cause disabling symptoms. ⋯ Patellar dislocation after total knee arthroplasty can cause disabling symptoms, including pain, weakness, limited range of motion, extension lag and difficulty when climbing up or down stairs. The cause of patellar dislocation after total knee arthroplasty was error in surgical technique in this series. Revision of the component should be performed in those with malaligned component. Proximal realignment is recommended in those with quadriceps imbalance. Distal realignment is recommended only when proximal realignment alone has failed to restore proper patellar tracking, this should be carried out with great caution due to the potential for serious complications.

Clinical TrialThe alveolar permeability in patients with diffuse infiltrative lung disease detected by Tc-99m DTPA radioaerosol inhalation lung scintigraphy and the relationships with the pulmonary function test.
In recent years, there has been growing interest in determining the solute exchange across the pulmonary epithelium for various diseases. Alveolar permeability (AP) in patients with diffuse infiltrative lung disease (ILD) was investigated in an attempt to correlate it with parameters of the Pulmonary Function Test. ⋯ When the AP of ILD patients is significantly damaged, however, the severity of the damage is not related to chest X-ray findings. Radionuclide alveolar permeability study is not related to traditional pulmonary function tests. The radionuclide alveolar permeability study should be used as a new method to evaluate lung function.

Use of noninvasive positive pressure ventilation via nasal mask in patients with respiratory distress after extubation.
Noninvasive positive pressure ventilation (NIPPV) has been successfully applied to provide ventilatory support in patients with chronic respiratory insufficiency as well as in selected patients with acute respiratory failure of various etiologies. To investigate the effectiveness of NIPPV via nasal mask in patients who develop respiratory distress after extubation, we prospectively studied 19 such patients (14 elective extubations and 5 self-extubations) who required no immediate reintubation. ⋯ NIPPV via nasal mask may be considered as an alternative to endotracheal reintubation in selected extubated patients with respiratory distress who require no immediate reintubation.

Randomized Controlled Trial Comparative Study Clinical TrialVecuronium dissolved in normal saline exaggerates pain on intravenous injection.
On some anesthetic occasions, many patients complained of pain at the site of injection when vecuronium, dissolved in normal saline, was administered intravenously. The aims of this study was to compare the incidence of pain on i.v. injection of vecuronium either dissolved in normal saline or in a solvent provided by the pharmaceutical manufacturer. ⋯ Pain resulting from intravenous injection of vecuronium can be aggravated if normal saline was used as the solvent. Increased osmolality may also be responsible for this phenomenon.

Case ReportsTorsion of an ovarian borderline tumor with extreme serum tumor markers: a case report.
A postmenopausal woman with torsion of the left ovarian mucinous cystadenoma with borderline malignancy is reported. Extremely elevated serum tumor markers such as carcinoembryonic antigen (CEA) (398.0 ng/mL, normal < 3.0 ng/mL), carbohydrate antigen 19-9 (CA 19-9) (240 U/mL, normal < 37 U/mL) and tissue polypeptide antigen (TPA) (1,918 U/L, normal < 110 U/L) were found preoperatively. Imaging studies such as ultrasound and computed tomogram favored a diagnosis of ovarian malignancy, and advanced ovarian cancer was suspected prior to operation. ⋯ Serum levels of the tumor markers became normal gradually following operation. To present knowledge, this is the first reported case with extremely high serum tumor markers in a postmenopausal woman with torsion of a stage-Ia ovarian mucinous cystadenoma of borderline malignancy. Rapid release of these tumor markers into circulation secondary to necrosis and degeneration of the tumor cells following torsion might account for such an unusual finding.
