Acute lobar nephronia (ALN), a term analogous to acute lobar pneumonia, refers to a renal mass caused by acute focal infection without liquefaction. An alternative term is acute focal bacterial nephritis (AFBN). Fever, flank pain or back pain are the most frequently encountered clinical characteristics. ⋯ Further distinction between acute lobar nephronia and other renal masses is aided by the appropriate use of renal sonography and computerized tomography is the most effective and least costly method in diagnosis. Serial sonograms can be used to monitor response to antibiotic therapy. Clinical and uroradiographic characteristics of a six year-old female patient with acute lobar nephronia are presented, along with a review of literature.

[Transtympanic electrocochleography in Menière's disease].
In order to examine the usefulness of the electrocochleography (ECoG) in the diagnosis of Menière's disease and establish the diagnostic criteria in Chinese, transtympanic ECoG was performed on 18 normal ears, 22 Menière's ears and 14 ears with cochlear type sensorineural hearing loss other than Menière's disease. Although the Menière's ears had larger mean summating potential (SP) than the normal ears, the difference between both groups was not statistically significant. ⋯ The SP/AP amplitude ratio of Menière's ears tended to increase while the mean hearing level of 4-8 KHz deteriorated. Also, abnormal enlargement of the SP/AP amplitude ratio in Menière's disease tended to be the result of decreased AP amplitude.

Randomized Controlled Trial Comparative Study Clinical TrialPostoperative pain relief: lumbar and thoracic epidural morphine in thoracotomy.
Twenty-two patients undergoing thoracotomy were enrolled in a controlled, randomized trial to compare the effects of thoracic (group T) or lumbar (group L) epidural morphine in relieving postoperative pain. Epidural morphine was given as the patient first complained of pain in the recovery room. The effect of epidural morphine on pain relief was assessed by visual analogue scale and subjective grade. ⋯ There was no significant difference between two groups with regard to the effect of morphine as assessed by subjective grading. We concluded that thoracic epidural morphine took effect faster than lumbar epidural morphine. However, the effects of both kept similar since 50 minutes after morphine injection.

Case ReportsBilateral traumatic chylothorax: a complication of fracture-dislocation of the T-spine.
Traumatic chylothorax is a rare complication of fracture-dislocation of the thoracic spine. Not treated adequately, it will be associated with high mortality and morbidity. A review of the English literature shows that bilateral traumatic chylothorax is rarely discussed. ⋯ The mechanism of injury is described. The management strategies are, initially, conservative treatment including chest tube drainage, NPO with total parenteral nutrition (TPN), followed by medium-chain triglyceride diet. Without improvement, surgical measures such as mediastinal ligation of the thoracic duct should be considered.

Case Reports[Pulmonary edema in transcervical endometrial ablation: a case report].
The use of the transcervical resectoscope for endometrial ablation and/or resection of myomas is a relatively new technique in gynecologic surgery. Although complications are uncommon with this procedure, they can occasionally appear and lead to potential hazard. ⋯ After administration of diuretics, sodium supplement and 100% oxygen, complications improved progressively. The patient was discharged on postoperative day 5 without any sequelae.
