Case ReportsMalignant meningioma with rhabdoid transformation.
We report a rare case of recurrent meningioma with malignant change and rhabdoid transformation in a 54-year-old woman who presented with severe headache and progressive weakness of the right extremities. The patient had a history of atypical meningioma and had undergone a craniotomy to remove a tumor nine years earlier. We discuss the distinctive morphologic, immunohistochemical staining and ultrastructural features of a recurrent malignant meningioma. A meningioma with rhabdoid transformation may indicate aggressive biologic and clinical behavior of the tumor.

Fluidity and fatty acid components of erythrocyte membranes in diabetic retinopathy.
Recently, numerous anomalies of erythrocyte rheology and function have been reported in diabetic patients. These changes are thought to be related to the alteration of erythrocyte membrane fluidity and may play an important role in diabetic retinopathy (DR). Nonetheless, the principal factors contributing to fluidity change remain undetermined. The influence of erythrocyte membrane fatty acid components on membrane fluidity and retinopathy was evaluated in patients with type 2 diabetes. ⋯ The results suggest that erythrocyte membrane fatty acid components may contribute to alterations in membrane fluidity in patients with type 2 diabetes. Alterations in membrane fluidity may play an important role in the development of diabetic microangiopathy.

Case ReportsSquamous cell carcinoma of the right upper lung congenital tracheal bronchus.
Tracheal bronchus, directly arising from the abnormal tracheal wall, is a rare congenital anomaly of the airway. Lung cancer developing in a tracheal bronchus is even more rare. A 68-year-old man had a mass shadow over the right upper lung on chest radiography. ⋯ The subsequent pathologic examination of the specimen revealed squamous cell carcinoma with surrounding pneumonitis. Bronchogenic cancers developing in a tracheal bronchus are rarely reported in the English language literature. Surgical resection appears to be the treatment of choice.

Effect of milrinone on postbypass pulmonary hypertension in children after tetralogy of Fallot repair.
Postbypass pulmonary hypertension in surgical correction of tetralogy of Fallot (TOF) is a risk for right ventricular failure. Effective management remains a major challenge. Milrinone is a new drug with a unique mechanism of "inodilation", which offers both inotropic and vasodilatory effects. We attempted to determine if application of milrinone could improve cardiopulmonary dysfunction in children after TOF repair. ⋯ We conclude that milrinone is effective in the management of pulmonary hypertension following CPB in children who underwent TOF repair.

Flickering light and color patterns, reading, language, movement, decision making, eating, tapping and touching, hot water immersion and auditory stimulation can induce seizures in some patients. This is known as "reflex epilepsy". The mechanism of reflex epilepsy is not clear. Recently, we cared for 12 patients (11 men and 1 woman, age 41-74 years; mean age = 63.1 years) whose seizures were induced by playing mah-jong, with mean age at seizure onset of 48.7 years. Nine of the 12 patients had seizures exclusively while playing mah-jong. ⋯ "Mah-jong epilepsy," a new reflex epilepsy, is probably related to thinking and decision making while playing mah-jong. The easy control of seizures induced by mah-jong suggests a benign nature. In addition to anticonvulsant therapy, avoiding playing mah-jong may be essential in preventing seizures.
