Acute pancreatitis in pregnancy is rare. Our purpose in this study was to discuss the etiology, incidence and course of pancreatitis in pregnancy and to evaluate the maternal and perinatal outcomes. ⋯ Early diagnosis and treatment is of utmost importance in the management of acute pancreatitis in pregnancy. The results of this study showed good maternal outcome following appropriate treatment. Fetal prognosis was less favorable and was most often associated with hyperlipidemia. Fetal monitoring is essential during the management of pancreatitis in pregnancy.

Clinical Trial Controlled Clinical TrialEffect of preoperative transdermal ketoprofen on post-hysterectomy pain.
In an attempt to reduce central sensitization after tissue injury, the concept of "pre-emptive analgesia" was evolved. The aim of the present study was to evaluate the analgesic effect of pre-incisional transdermal ketoprofen on post-hysterectomy pain. ⋯ It seems that pre-incisional transdermal ketoprofen (30 mg, twice the day before surgery) has only a limited pre-emptive effect on post-hysterectomy pain as indicated by delayed first morphine request. Further, it had no effect on postoperative VAS, morphine consumption or incidence of nausea and vomiting.

Case ReportsLeptomeningeal malignant melanoma arising in neurocutaneous melanocytosis: a case report.
A rare case of histology-proved giant congenital melanocytic nevus (GCMN) with symptomatic leptomeningeal melanocytosis is reported. A 26-year-old man had had a large patch of pigmented nevus over his back and left arm since birth. He had begun to have seizures as well as symptoms and signs of increased intracranial pressure about six months before admission. ⋯ Biopsy on one of the intracranial nodules revealed malignant melanoma arising in the melanocytosis. He died one year after the onset of neurologic symptoms. For early diagnosis of neurocutaneous melanocytosis, we suggest 1) MRI, and 2) leptomeningeal biopsy in patients with suspected leptomeningeal malignant melanoma.

Visual field defects in patients with pituitary adenomas.
The most common objective manifestations of pituitary adenoma are visual, including impairment of a visual field and loss of acuity. The characteristic visual field defect caused by pituitary adenomas with suprasellar extension is bitemporal hemianopsia. The present study was undertaken to determine the relationship between a visual field defect and the chiasma compression caused by pituitary adenomas. ⋯ There was a linear correlation between a visual field defect and the chiasma compression no matter whether measured from computed tomography or from MR imaging. The visual field defect occurred mostly when the chiasma compression exceeded a certain level. A more severe visual field defect appeared in patients with nonfunctional pituitary adenoma or those with a prior operation history for the pituitary adenoma.

Randomized Controlled Trial Clinical TrialImproved postoperative pain relief via preemptive analgesia in relation to heart rate variability for coronary artery bypass grafting: a preliminary report.
Sudden cardiac death remains to be the most devastating outcome for patients who undergo cardiac surgery. Surgical stress in combination with postoperative pain are key factors of such tragedy. Preemptive analgesia (PA) is supposed to provide optimal postoperative pain management, and epidural morphine infusion to provide effective pain relief and stress reduction after cardiac surgery. Heart rate (HR) variability seems to correlate to the survival of acute myocardial infarction (AMI) patients. This study was conducted to evaluate the benefits of PA for coronary artery bypass grafting (CABG) patients in the sense of improvement in postoperative respiratory and cardiovascular functions, and its correlation to HR and HR variability. ⋯ PA method through the use of epidural anesthesia along with morphine infusion provided better postoperative pain relief and more stable respiratory and cardiovascular functions in our patients with cardiac surgery. Although the differences in HR and HR variability between the control and the PA groups were not statistically significant throughout the period studied, the potential benefits of better understanding the relationship between postoperative pain control and HR variability justify further studies to be conducted.
