The clinical syndrome of penetrating injuries to the buttocks at the Medical College of Virginia has been reviewed. This syndrome may present with a spectrum of injuries, from minor soft tissue damage to serious bowel, genitourinary, or vascular damage. ⋯ Additional diagnostic studies such as proctoscopy and cystoscopy are helpful when additional preoperative information is required. When retroperitoneal and intraperitoneal injury is suspected, prompt exploration with aggressive intraoperative management is recommended.
