Randomized Controlled TrialComparison of Effect of Levocetirizine or Montelukast Alone and in Combination on Symptoms of Allergic Rhinitis.
Objective. To determine the effect of levocetirizine, montelukast and the combination of both the drugs on symptoms of allergic rhinitis. Methods. Seventy-five patients with allergic rhinitis were randomly studied prospectively for a period of two weeks divided into three groups receiving either levocetrizine or montelukast or combination of both. ⋯ There was also no difference in the control of night-time and eye symptoms among the regimens. Conclusions. Montelukast, levocetirizine and their combination is equally effective in controlling symptoms, of allergic rhinitis. Thus, use of montelukast alone would be cost-effective and do avoid adverse effects of levocetirizine in combination therapy

Case ReportsSpectrum of Clinical Presentation of Pulmonary Alveolar Microlithiasis: Case Series from North India.
We describe five cases of pulmonary alveolar microlithiasis (PAM) from North India with characteristic radiological and histopathological features. All patients were symptomatic with variable severity and duration of the symptoms and one patient developed cor-pulmonale during the course of follow-up. Diagnosis of PAM was suspected on the basis of classical sand-storm appearance on radiological examination and confirmed by transbronchial lung biopsy in four of the five cases. Apart from other features, presence of pleural and pericardial calcification in one case makes this discussion valuable. Awareness of this specific entity among the clinicians is essential to avoid unnecessary investigations.

Observational StudyClinical Profile of Spontaneous Pneumothorax in Adults: A Retrospective Study.
Pneumothorax is an important cause of morbidity and mortality, but limited data are available regarding the aetiology, clinical profile, management and outcome of spontaneous pneumothorax from India in recent times. ⋯ Our observations suggest that pneumothorax is more common among men. Secondary spontaneous pneumothorax is more common than PSP and the most common cause of SSP was pulmonary TB followed by COPD.

Swyer-James-MacLeod syndrome is a radiological entity characterised by hyperlucency of one or more lobes or of the entire lung associated with decreased number and diameter of ipsilateral peripheral pulmonary vessels resulting in difficult visibility of the arterial network, and unobstructed bronchial system. We report the case of a 12-year-old girl who presented with chief complaint of pain in the chest. ⋯ Contrast-enhanced computed tomography (CECT) of the chest showed hyperlucency and diminished vascularity in the upper lobe and lingula of left lung with hyperinflation of the pulmonary parenchyma. On the basis of clinical and radiological findings, the patient was diagnosed to have Swyer-James-MacLeod syndrome.
