Review[Epidemiology, treatment and prevention of type 2 diabetes in children and adolescents].
Changes in food consumption and exercise are fueling a worldwide increase in obesity in children and adolescents. As a consequence of this dramatic development an increasing rate of type 2 diabetes can be observed in children and adolescents in many countries. Development of strategies for therapy of type 2 diabetes and implementation of prevention of obesity and type 2 diabetes in this age group should be primary health care goals.

Case Reports[Recurrent syncope after blunt trauma of the thorax].
A 35-year-old patient presented with a non-penetrating chest trauma due to an automobile accident. Examinations showed a trauma of the left shoulder (reversed Hill-Sachs lesion). After diagnostic procedures, he underwent surgery four weeks later. During the hospital stay, the patient developed angina pectoris, dizziness and syncopes. ⋯ ECG monitoring after blunt chest trauma in the early period and after several days is mandatory to screen and prevent potentially life threatening posttraumatic arrhythmias.

Case Reports[Lymphangioleiomyomatosis (LAM): a rare cause of ascites and pleural effusion].
A 45 year old woman presented at our hospital for further evaluation of intermittent abdominal pain. The patient reported that she had suffered a spontaneous pneumothorax 8 years and a pleural effusion 3 years before. ⋯ This report depicts a case of LAM which involved both pulmonary and intestinal symptoms. Previous pulmonary problems gave a hint towards the right diagnosis. Though lymphangiography is rarely performed in these days, the method was both diagnostic and therapeutic in the case presented.

[Health care ethics committees in German university clinics. A survey of all medical directors and directors of nursing].
Health Care Ethics Committees (HEC) are new ways of implementing clinical ethics in hospitals and nursing homes. In the US all health care institutions must provide some structure to handle ethical conflicts in everyday patients' care. However, in Germany only a minority of hospitals, often members of the Protestant or Catholic Hospital Associations, have founded HEC. No studies exist in regard to the situation at German university hospitals. ⋯ Our data show a high need for information about HEC in German university hospitals. The tasks, working methods and chances for further development of HEC are neither known nor used in the majority of German university hospitals.
