Ventilator-associated pneumonia after pediatric cardiac surgery in southern Taiwan.
To determine the frequency, risk factors, associated pathogens, and outcomes of ventilator-associated pneumonia (VAP) after pediatric cardiac surgery. ⋯ VAP is common after congenital heart surgery. Physicians must pay special attention to infants with complex CHD because they are at high risk for the development of VAP after congenital heart surgery. Shortening the duration of mechanical ventilation and central venous catheter placement are critical factors for reducing the risk for VAP.

Activity of ertapenem, ciprofloxacin, ceftriaxone, piperacillin-tazobactam, and ampicillin-sulbactam against 12 common clinical isolates of community-acquired bacteremia.
To compare the antimicrobial activities of ertapenem, ciprofloxacin, ceftriaxone, piperacillin-tazobactam, and ampicillin-sulbactam against 12 common organisms that cause community-acquired bacteremia and to identify the most active agents for the treatment of extended-spectrum beta-lactamase (ESBL)-producing Escherichia coli and Klebsiella pneumoniae. ⋯ ESBL E. coli and K. pneumoniae are highly resistant to ciprofloxacin, so this antibiotic should be avoided for patients with community-acquired bacteremia. ESBL E. coli and K. pneumoniae are highly susceptible to ertapenem.

The role of antimicrobial therapy for treatment of uncomplicated skin and soft tissue infections from community-acquired methicillin-resistant Staphylococcus aureus in children.
Community-acquired methicillin-resistant Staphylococcus aureus (CA-MRSA) infections in skin and soft tissues are increasing in children in Taiwan. This study investigated the outcomes of therapy with or without appropriate antibiotics among children with CA-MRSA skin and soft tissue infections (SSTIs), and analyzed the outcomes of management among children with Panton-Valentine leukocidin (PVL)-positive strains and PVL-negative strains. ⋯ The high rate of successful treatment among children with uncomplicated CA-MRSA SSTIs, even when given inappropriate antibiotic therapy, suggests that treatment of these uncomplicated infections without appropriate antibiotic therapy is possible. Incision and drainage may play an important role in the treatment of uncomplicated SSTIs.

Genetic fingerprinting and antimicrobial susceptibility profiles of Pseudomonas aeruginosa hospital isolates in Malaysia.
Pseudomonas aeruginosa is the third most common pathogen causing nosocomial infections. The objective of this study was to investigate the antimicrobial resistance profiles and genetic diversity of hospital isolates of P. aeruginosa and to investigate the presence of several resistance genes and integrons. ⋯ Aztreonam appeared to be the most effective agent against multidrug-resistant P. aeruginosa isolates. Sixty nine percent of the P. aeruginosa isolates analyzed were multidrug resistant and the isolates were genetically diverse.

ReviewMicrobiologic spectrum and susceptibility pattern of clinical isolates from the pediatric intensive care unit in a single medical center - 6 years' experience.
Nosocomial infection contributes to the morbidity and mortality of patients in hospital. Timely microbiologic surveillance and assessment of antimicrobial resistance is important for dealing with nosocomial infections. This retrospective review was conducted to evaluate the microbiologic spectrum and susceptibility pattern in the pediatric intensive care unit from 2001 to 2006. ⋯ MRSA, carbapenem-resistant P. aeruginosa, and ESBL-producing K. pneumoniae were the major concerns in this study. Good hand hygiene and strict aseptic procedures remain the most important factors for infection control.
