Both pulmonary lymphangioleiomyomatosis and tuberous sclerosis are rare diseases. The optimal management in pregnancy is unclear. ⋯ Pulmonary lymphangioleiomyomatosis is a consideration in tuberous sclerosis patients with respiratory symptoms. Tuberous sclerosis patients with pulmonary lymphangioleiomyomatosis require cautious and calculated expectant management in an effort to avoid adverse outcomes.

Is antibody screening in Rh (D)-positive pregnant women necessary?
Hemolytic disease of the fetus and of the newborn can be caused by incompatibility of maternal and fetal erythrocytes for Rh (D) or other blood type antigens. Routine antibody screening is advocated in all pregnant women, irrespective of whether they are Rh (D)-positive or Rh (D)-negative, to look for clinically significant alloantibodies other than Rh (D) that might cause hemolytic disease of the newborn. The purpose of this study was to assess the incidence of blood type antibodies other than Rh (D) in pregnant women attending for prenatal care in a typical urban population. ⋯ Routine antibody screening of Rh (D)-positive women is probably not warranted from a clinical cost-benefit perspective.

To determine the rate of bioavailability of oral misoprostol in the tablet and a new capsule form in women with term pregnancies in the postpartum period. ⋯ Oral misoprostol is rapidly absorbed and bioavailable in the postpartum period. Misoprostol may prove useful in postpartum management. The novel packaging of misoprostol in capsule form allows for double-blinded studies with similar pharmacokinetics to the standard tablet.

To determine the effects of labor induction with vaginal prostaglandin E2 (PGE2) on mode of delivery in post-term pregnancies and to identify possible predictors of successful vaginal delivery. ⋯ PGE2 induction of labor was successful in approximately 80% of patients with post-term pregnancy, with apparently no serious maternal or fetal complications. Risk factors intrinsic to the patient, and labor induction itself, were the reasons for excess Cesarean deliveries in women with prolonged pregnancies.
