[The 3-hour meeting concept: an organisational approach for involvement of junior doctors in education].
In an 850-bed Danish University hospital, "3-hour meetings" provide junior doctors with three hours within normal working hours for discussing training with an innovative purpose. The meetings are followed-up by dialogue and action throughout the organisation. ⋯ The Head of Department obtains information, which is used for prioritisation of educational initiatives. At hospital level, an overview of education as well as sharing of knowledge and educational vision is achieved.

Career counselling is meant to support and ensure an early and relevant choice of specialty. Self-awareness regarding personality, life goals, wishes for family life, and lifestyle is of help in narrowing down the number of specialties to those that fit personal attitudes and preferences. The counsellor must be aware that the trainees' subjective opinions about the specialties may not be in line with the actual conditions. Hence, career counselling should provide factual knowledge about the specialties including information on the working conditions and defining characteristics of the specialties.

Before the new temporary ice rink in Esbjerg was built, opponents feared that it would disproportionately increase the number of traumas needing medical care at the hospital. The reason for this concern was that it was built with ramps. ⋯ Having ramps on the ice rink produced no significant increase in the proportion of injuries, nor did it produce more severe traumas. Safety equipment protecting hand and wrist would probably decrease the number of soft tissue injuries presented at hospital.

New clinical findings suggest a connection between Amyotrophic Lateral Sclerosis (ALS) and Frontotemporal Dementia (FTD). The two diseases are often seen simultaneously, which complicates the course of the disease for the ALS-patient and the relatives. This paper illustrates this problem and describes a way to monitor the ALS-patient's cognitive and personal changes in an effort to improve his or her rehabilitation and palliation.

Severe metabolic acidosis is associated with poor prognosis. We present a patient with profound alcohol and starvation-related combined lactic and keto acidosis (lactate = 29 mM; pH = 6.83) who made a good recovery following 18 hours of intensive care therapy. A brief summary of the proposed mechanism by which these metabolic derangements develop is presented.
