Case Reports[The clinical course of a three year-old girl after overdose of valproic acid].
Valproic acid (VA) is an antiepileptic drug, and frequently prescribed in pediatrics. VA overdose results in central nervous system depression, insufficient respiration, tachycardia, hypotension, hepatotoxicity, and electrolyte derangement. ⋯ Clinical observation included measurements of blood pressure, cardiac telemetry, and measurements of serum VA and metabolic parameters every 4-6 hrs. The girl was discharged successfully after 72 hrs with her usual dose of VA.

[High standards of educational climate in Danish anaesthesiological departments].
For the first time since the implementation of the Danish national medical educational reform in 2004 the effects on the educational programme are investigated by means of a nation-wide questionnaire. Mandatory concepts introduced with the educational reform such as supervisor involvement, appraisal meetings and personal learning plans are almost completely implemented in the anaesthesiological departments. ⋯ The educational climate is reported to be of high standards. Hopefully, this survey can inspire other specialities to identify how education can be improved.

Case Reports[Cardiac tamponade and purulent pericarditis caused by Streptococcus pneumoniae].
Purulent pericarditis is a rare entity in the modern antibiotic era. Despite advances in treatment and diagnostics, purulent pericarditis remains a life-threatening disease. ⋯ The patient was treated successfully with drainage and intravenous penicillin. Due to the high mortality of this disease, early diagnosis, aggressive pericardial drainage and appropriate antibiotic therapy are essential.

Hyponatraemia with cerebral symptoms is a medical emergency. Prompt management of airway, breathing and circulation together with repeated boluses of 2 ml/kg 3% NaCl constitute a rational treatment. ⋯ P-[Na(+)] and diuresis must be measured frequently together with diuresis. Definitive treatment should be directed toward the aetiology.

Review[Varying effect of telemedicine in the treatment of chronic obstructive pulmonary disease--a systematic review].
A systematic review of existing evidence on the efficiency of telemedicine solutions for patients with chronic obstructive pulmonary disease (COPD) was performed. A systematic literature search was conducted in five relevant databases followed by evaluation of methodology and results in selected studies. ⋯ Benefits of using telemedicine solutions for patients with COPD are not yet clear, but in some multimodal studies with telemedicine as part of the solution a positive effect on hospital admissions, emergency room visits and contacts to primary care may be seen. Larger studies are needed.
