[Pulmonary hypertension and heart failure: the role of pulmonary vasculature].
Left heart disease is the most common cause of pulmonary hypertension. Increased left-sided filling pressure leads to passive postcapillary venous hypertension. In some patients, pulmonary vasoconstriction and vascular remodeling may lead to a further increase in pulmonary pressure. ⋯ The contribution of precapillary PH in diastolic heart failure is not known but can be significant and lead to disproportionate PH particularly in elderly. The precapillary component of pulmonary hypertension could be a therapeutic target for specific pulmonary vasodilators. Until now pharmacological trials has been disappointing and those medications can be dangerous because of increasing blood flow to the pulmonary capillaries with a risk of pulmonary edema when left sided pressure are still elevated.

[Acute heart disease in search and rescue helicopter missions, from French Navy. A retrospective study from 2000 to 2007].
Search and rescue helicopters from the French navy conduct ambulance and search and rescue missions near the western coast of the French Britain. The team on board includes military doctor and paramedic. Operations in this area are challenging due to long distances and severe weather conditions. ⋯ Ambulance and search and rescue mission near the western coasting of the French Britain are a challenge. Using a heavy helicopter from French Navy was the best way to carry medical treatment to seafarers and passengers of ferries suffering from acute heart disease.

Since the introduction of routine assay for natriuretic peptides, there are a growing number of clinical applications for those new tests. Numerous studies have defined analytical characteristics and clinical interest of NT-proBNP assay. Originally limited to acute heart failure diagnosis in the emergency room, NT-proBNP assay has now a wide number of applications. This literature review presents the "state of art" of this marker, detailing NT-proBNP physiological recent knowledge and its recognized or investigated clinical applications.

Case Reports[Night cardiac rhythm and conduction disturbances revealing obstructive sleep apnoea syndrome].
There is a strong prevalence of sleep apnoea syndrome in general population. Cardiac arrhythmia and conduction disturbances during sleep may complicate this syndrome. We report the case of a 73-year-old patient to whom the sleep apnoea syndrome was diagnosed after varied heart blocks during the sleep. A treatment by continuous positive airway pressure (CPAP) permitted to correct these conduction disturbances and to avoid pacemaker implantation.

Review[Chronic renal insufficiency and cardiovascular disease].
Renal insufficiency is frequently seen in patients with cardiovascular disease. In contrast, coronary artery disease is the leading cause of death in patients with renal impairment. The recognition of renal insufficiency is essential in these patients and preventive measures must be put in place to prevent the progression or onset of cardiovascular disease. In this article, we explain the methods to assess kidney function, the epidemiology of coronary heart disease in patients with renal impairment, risk factors conventional and non-conventional found in these patients and the main recommendations for their therapeutic care.
