[The Berlin reformed medical curriculum at the Charité. Experiences with the first cohort].
At the end of the 2004 summer term the first students of the Berlin reformed curriculum finished their education and took their first state examination. With regard to the questions to be answered by the "Berlin Model" the results of the first cohorts were presented and discussed. Overall the Berlin curriculum was successful. The results led to some important considerations concerning adaptation by other faculties, the requirements to start off a problem-based curriculum or an enlargement of the class.

[New licensing regulations for physicians. Main areas of reform and first results of the implementation process].
Since October 2003 medical education in Germany has been given a new more up-to-date basis, the "New Licensing Regulations for Physicians". They represent a reform of both the structure and the content of medical education making great demands on medical faculties and medical students; e.g. elective courses during the first and second cycles of the curriculum, interdisciplinary courses (so-called 'cross-sectional courses'), practical clinical courses of several weeks in five specialties, and family medicine as an option for choice during the last year are central issues of the reform. The number of state-controlled medical examinations has been reduced from four to two. ⋯ This paper presents the essential innovations and describes the current state of the implementation process. It also points out problems that deserve reconsideration. Although the full implementation of the new licensing regulations is still in progress, it might be said that they have already initiated important changes in medical education in Germany.

[Health economics and HTA].
For a comprehensive health technology assessment (HTA), health economic studies are analysed as well as medical studies. Significance and effects of HTA are rather different in European countries. In Germany HTA is conducted by two official agencies: DIMDI and IQWiG. ⋯ For this decision medical, utilitarian or other aspects like the avoidance of unwanted distribution effects are considered. On the other hand often only a few studies are available for the decision making when the appraisal has to be done. This problem might be less relevant in the near future as the number of HTAs and the number of health economic evaluations increases.

Comparative Study[The German Medical Association support initiative on health care research].
The 108th German Medical Assembly adopted a 6-year support initiative for health care research in order to contribute to the further development of this young scientific discipline which has not yet been sufficiently established in Germany. The program is focussed on the following subjects: (1) influence of economic changes on outpatient and inpatient medical care, (2) implementation of medical guidelines in medical routine and (3) the physician factor. Setting methodical standards in this new field and opening up access to medical data should provide a relevant impulse to health care research in Germany. The promoting strategy of the German Medical Association will be completed by initiating a network of scientists, research promoters and health care players.

Comparative Study[Methods in health services research. The example of the evaluation of the German disease management programmes].
According to the established definition of Pfaff, health services research analyses patients' path through the institutions of the health care system. The focus is on development, evaluation and implementation of innovative measures of health care. By increasing its quality health services research strives for an improvement of efficacy and efficiency of the health care system. ⋯ For the purpose of managing its tasks adequately a special combination of instruments and methodological procedures is needed. Thus, diverse techniques of evaluation research as well as special requirements for study designs and assessment procedures are of vital importance. The example of the German disease management programmes illustrates the methodical requirements for a scientific evaluation.
