Occupational health in small scale and household industries in Nepal: a situation analysis.
Making working conditions safe and healthy is the interest of workers, employers and the Government.Although it seems simple and obvious, this idea has not yet gained meaningful recognition in Nepal. ⋯ The occupational health and safety practices in small scale industries in Kathmandu have been found to be unsatisfactory. Child labour is a serious problem. Out of ten industries, six have employed child workers and the working conditions range from bad to terrible. Health and welfare of the child workers was also not satisfactory.

Medical schools and medical education look different as we advance into the 21st century. Nepal has seen a dramatic increase in the number of medical schools/colleges in the last decade. Most schools practice traditional teaching method while others are implementing problem based learning (PBL). It is important to explore the current advances and practices in medical education to meet the needs of the health services of the country. ⋯ The medical education of Nepal is still guided by the notion of the traditional approaches. The teaching methods should come up as per the advancement in contemporary medical education. The training needs to be more structured and focused in practical reality than only feeding students with theoretical knowledge.

Randomized Controlled TrialOral administration of intravenous solution of midazolam mixed in syrup of paracetamol is an effective way of premedicating children undergoing surgery under general anaesthesia.
The purpose of this study was to evaluate the efficacy of injection midazolam administered by oral route mixed in paracetamol syrup as a premedication in children undergoing surgery. ⋯ It was concluded that injection midazolam mixed in syrup paracetamol administered orally is a convenient and efficient method of premedicating children undergoing general anaesthesia. Parent-child separation and induction of anaesthesia was smooth and the recovery uneventful in children premedicated with oral midazolam.

Application of "the Sequential Organ Failure Assessment (SOFA) score" in predicting outcome in ICU patients with SIRS.
Various scoring systems have been developed to prioritize patient admission and management in ICU. The objective of this prospective, observational cohort study was to evaluate application of one such system, the Sequential Organ Failure Assessment (SOFA) Score in predicting outcome in ICU patients with SIRS. ⋯ The SOFA scoring system is useful in predicting outcomes in ICU and thus help in proper utilization of ICU resources.

Case ReportsSecondary extramedullary plasmacytoma of the breast: diagnosed by fine needle aspiration cytology.
A 34years old female who was a known case of multiple myeloma presented with a mass in the right breast. Clinically a differential diagnosis of breast carcinoma or secondary involvement by multiple myeloma was considered. Fine Needle Aspiration Cytology (FNAC) of the mass was performed which showed dispersed population of plasma cells with eccentrically placed nuclei, abundant cytoplasm and the characteristic paranuclear hof.
