Insomnia and excessive sleepiness are common in the investigation of sleep-disordered breathing. Circadian rhythm sleep disorders are perhaps the most often overlooked conditions in the differential diagnosis of these symptoms. Circadian rhythm sleep disorders manifest as misalignment between the sleep period and the physical/social 24-h environmental cycle. ⋯ However, there are nine possible diagnoses, and all nine are of clinical interest. Since light is the principal cue used in synchronizing the biological clock, blind individuals and night-shift/swing-shift workers are more prone to develop circadian rhythm sleep disorders. In this article, the new international classification of circadian rhythm sleep disorders is reviewed.

Pulmonary hypertension is a common complication of sickle cell anemia. Despite the fact that the elevations in pulmonary artery pressures are slight, morbidity and mortality are high. In adult sickle cell anemia patients, pulmonary hypertension is emerging as a major risk factor for death. ⋯ Specific data on the effects of treatment modalities for pulmonary hypertension in patients with sickle cell anemia are scarce. It is likely that all patients would benefit from maximization of sickle cell anemia therapy, and that patients with the severe form of the disease would benefit from treatment with selective pulmonary vasodilators and antiproliferative agents. Large trials evaluating the effects of treatment for pulmonary hypertension in the sickle cell anemia population are underway.

Chronic obstructive pulmonary disease is a progressive and debilitating disease that is typically diagnosed only after a long period of gradual worsening. Dyspnea is the symptom that most often interferes with the execution of professional, family, social and daily-life activities of patients with chronic obstructive pulmonary disease. Such limitations can lead to a sedentary lifestyle and worsen overall quality of life. ⋯ Even simple tasks can result in high oxygen consumption and minute ventilation, which accounts for the sensation of dyspnea reported by the patients. In view of these facts, it is appropriate to evaluate the impact that such incapacity has on daily life in patients with chronic obstructive pulmonary disease. Techniques of energy conservation that can be used as tools to minimize the discomfort of such patients are herein discussed, and those considered most appropriate are highlighted.
