Studies of older and less active patients with obstructive sleep apnea (OSA) have reported decreased exercise capacity as measured by peak oxygen uptake (VO2 max) during cardiopulmonary exercise testing (CPET). We looked to determine whether VO2 max was decreased in younger patients with OSA who regularly exercise as would be encountered in the military. ⋯ A commentary on this article appears in this issue on page 819.

Restless legs syndrome is a chronic disorder that is characterized by an urge to move the legs and unpleasant sensations in the lower extremities. Its symptoms develop slowly and a sudden onset is very unusual, which may be confusing for the emergency physician. ⋯ The patient shortly after infusion of metoclopramide started to experience rapid movements of the legs, claiming that it was the only way to relieve extremely unpleasant sensations in her legs. The symptoms subsided after saline infusion and did not appear again.

Previous studies have shown that non-rapid eye movement (NREM) sleep parasomnias commonly coexist with restless legs syndrome (RLS) and periodic limb movement disorder (PLMD) in children, leading to speculation that RLS/PLMD may precipitate or worsen parasomnias. However, there are limited data about the effect of the treatment of RLS/PLMD on parasomnias in children. Hence, we performed this study to determine whether the treatment of RLS/PLMD with oral iron therapy is associated with improvement of parasomnias in children. ⋯ Parasomnias are common in our cohort of children with RLS/PLMD. Iron therapy was associated with a significant improvement in periodic limb movement index, RLS symptoms, and resolution of a significant proportion of NREM sleep parasomnias, suggesting that RLS/PLMD may precipitate NREM sleep parasomnia.

A Global Comparison of Anatomic Risk Factors and Their Relationship to Obstructive Sleep Apnea Severity in Clinical Samples.
Obstructive sleep apnea (OSA) is a global health issue and is associated with obesity and oropharyngeal crowding. Global data are limited on the effect of ethnicity and sex on these relationships. We compare associations between the apnea-hypopnea index (AHI) and these risk factors across ethnicities and sexes within sleep clinics. ⋯ We demonstrate ethnic and sex variations in associations between obesity and OSA. For similar BMI increases, South American patients show greatest AHI increases compared to African Americans. Findings highlight the importance of considering ethnicity and sex in clinical assessments of OSA risk.
