This study was designed to evaluate inter and intraobserver concordance in gross tumor volume (GTV) delineation on megavoltage CT (MVCT) images in patients with postoperative vault recurrences. ⋯ Moderate to good inter and intraobserver GTV correlation was observed on MVCT images, however, was associated with low interobserver spatial concordance on both MVCT and CECT images. Strategies to improve contouring reproducibility on MVCT and KVCT images are desirable.

The role of invasion as a prognostic factor in high-grade gliomas (HGG) remains controversial. An apparent increase in invasiveness following anti-angiogenic therapy makes this question clinically relevant. The goal of this study is to assess survival differences in patients with newly diagnosed HGG who present with diffuse invasive disease compared to those who did not, but went on to develop diffuse invasive disease following bevacizumab therapy. ⋯ Presence of diffuse invasive disease not accompanied by angiogenesis either prior to therapy or subsequent to anti-angiogenic therapy does not seem to have prognostic significance. However, invasion accompanied by angiogenesis in newly diagnosed HGG may confer a poor prognosis.

Lactic acidosis (LA) due to malignancy was first reported in patients with acute leukemia. Since then, several malignancies have been reported to be associated with LA. The pathophysiology of cancer-related LA is multifactorial and still poorly understood. ⋯ LA is rare in patients with malignancies and is usually associated with high mortality because of advanced disease process and high tumor burden. Increased awareness of this complication in certain malignancies is important because early initiation of chemotherapy may decrease LA and perhaps prolong survival. To our knowledge, this is the first case of otherwise unexplained severe LA in a patient with chemotherapy-refractory metastatic prostate cancer.

Assess the diagnostic yield of real-time bronchoscopic ultrasound transbronchial needle aspiration (EBUS TBNA) in conventional TBNA-negative mediastinal lymphadenopathy and hilar lymphadenopathy. ⋯ EBUS TBNA has a good diagnostic yield in hilar lymphadenopathy and in conventional TBNA-negative mediastinal lymphadenopathy.
