Today, microsurgery plays a fundamental role in plastic surgery, expanding surgical possibilities for cancer, trauma and aesthetic-related surgery. However, throughout the years, teaching and training systems for microsurgery have not evolved in the same manner as its operative techniques. ⋯ However, stringent laws for animals used in experiments, combined with the rising costs of this model, have instigated the need to find alternative methods. To overcome this dilemma, we have developed a three-step approach to optimise microsurgery training, particularly when there is no access to animal laboratories.

Comparative StudyPosterior interosseous free flap for finger re-surfacing.
The authors present their clinical experience and surgical methods of soft-tissue coverage for the finger using a posterior interosseous free flap. ⋯ The posterior interosseous free flap may become one valuable option for finger re-surfacing. Avoiding the sacrifice of a main artery of the hand and shorter pedicle can reduce the morbidity and operative time associated with this procedure. In addition, separate regions can be reconstructed with a dual-paddle flap.

Necrotising fasciitis is a rare, life-threatening infection where there is necrosis of fascia, subcutaneous tissues and skin. Three adult patients presented with this condition affecting the extremities and all had predisposing medical factors. Each one complained of severe pain that seemed disproportionate to the skin affected. ⋯ These cases highlight the increasing incidence and differences in presentation of necrotising fasciitis, as well as the important symptom of pain. Patients can become moribund and require intensive care support. High mortality rates are well recognised.

Comparative StudyManagement of microstomia in adult burn patients revisited.
This study aims to review our experience in the surgical management of microstomia following facial burns. ⋯ Commissuroplasty is an early functional post-burn corrective procedure that often must be performed prior to completion of scar maturation. Mucosal advancement flaps are a viable procedure for the treatment of microstomia after facial burns, resulting in good aesthetic and functional outcome. Direct scar excision and skin grafting, although unavoidable in cases of extensive perioral scarring, frequently produces inferior results.
