The arterial partial pressure of carbon dioxide (PaCO2) represents the balance between CO2 production and consumption. Abnormal increase or decrease in PaCO2 can affect the body's internal environment and function. Permissive hypercapnia has aroused more attention as a novel ventilatory therapy. The aim of this study was to elucidate the effects of hypercapnia and hypocapnia on the functions of such neonatal organs as the lung and brain. ⋯ Severe hypercapnia and hypocapnia can cause neonatal brain injury and lung injury. Permissive hypercapnia can increase the survival of neonates with brain injury or respiratory system disease, and lessen the brain injury and lung injury caused by mechanical ventilation. However, the mechanism of permissive hypercapnia needs further exploration to confirm its safety and therapeutic utility.

To report a case of fatal tension pneumocephalus in a 9-year-old boy following a severe motor vehicle accident. ⋯ A high index of suspicion for tension pneumocephalus is required in patients with severe head injuries presenting with periorbital swelling and perioccular trauma. A prompt CT scan and neurosurgical intervention are indicated.

Arterial blood gas (ABG) analysis is the major tool for proper diagnosis and treatment of acid-base imbalance, but the invasive nature of arterial puncture and its possible hazards such as arterial spasm have resulted in a worldwide trend toward less-invasive diagnostic methods including venous blood gas (VBG) analysis. This study aimed to evaluate the validity of VBG and its clinical agreement with ABG in the 10 most common diseases in pediatric intensive care unit (PICU), and to answer how far it can replace the ABG test. ⋯ VBG can be used instead of ABG in some diseases such as respiratory distress syndrome, neonatal sepsis, renal failure, pneumonia, diabetic ketoacidosis and status epilepticus, but in other diseases such as neonatal seizure, shock, congestive heart failure and congenital heart diseases, ABG is preferable and must not be replaced by VBG. These results may be used for the formulation of future guidelines for PICU.

Enflurane requirement for blocking adrenergic responses to incision in infants and children.
Enflurane is one of the most commonly used inhaled anesthetics in China, but its requirement to block adrenergic responses after skin incision in pediatric patients is still unknown. This study was to determine the minimum alveolar anesthetic concentration (MAC) of potent inhaled anesthetics required to blunt the adrenergic response to skin incision of enflurane (MACBAR) in infants and children. ⋯ MACBAR of enflurane in infants older than 6 months is similar to that in young children. The MACBAR of enflurane decreases with co-administration of fentanyl in the pediatric population.
