Comparative Study
Intimate partner violence during the first pregnancy: A comparison between adolescents and adults in an urban area of Iran.
There is uncertain evidence that intimate partner violence (IPV) during pregnancy is more common among adolescents. We aimed to compare prevalence and chronicity of IPV during the first pregnancy between adolescents and adults. 136 women aged 15 to 19 and 272 women aged 20-29 years between 24 and 30 weeks gestation (stratified by center) were examined at all 80 public health centers/posts in Tabriz-Iran. IPV was assessed using the revised conflict tactics scales. ⋯ Using only two physical assault and sexual coercion subscales, rate of IPV perpetration fell to 40% vs. 28%, p = 0.016 and victimization fell to 46% vs. 38%, p = 0.227. There were no statistically significant differences between the groups in terms of prevalence and chronicity of various types of IPV, except sexual coercion victimization which was more prevalent among the adolescents (31% vs. 21%, p = 0.034). The high rates of IPV perpetration and victimization during pregnancy among both adolescents and adults in the study area with significant higher risk of sexual coercion victimization among adolescents require health policy makers and care providers to have serious efforts for its reduction.

To describe the prevalence, risk factors, signs and symptoms of non-fatal strangulation (NFS) in women referred to a Sexual Assault Resource Centre (SARC) following recent sexual assault. ⋯ This study identifies and quantifies NFS risk factors in female sexual assault and highlights the strong association with intimate partner sexual assault. Greater awareness of NFS in sexual assault should lead to improvement in medical screening, forensic management and safety risk assessment by sexual assault and domestic violence services, emergency departments and police.

Despite research demonstrating the overall safety of Conducted Electrical Weapons (CEWs), commonly known by the brand name TASER(®), concerns remain regarding cardiac safety. The addition of cardiac biomonitoring capability to a CEW could prove useful and even lifesaving in the rare event of a medical crisis by detecting and analyzing cardiac rhythms during the period immediately after CEW discharge. ⋯ An ECG recording device was successfully incorporated into a standard issue CEW without impeding the functioning of the device. This serves as proof-of-concept that safety measures such as cardiac biomonitoring can be incorporated into CEWs and possibly other law enforcement devices.

Sexual violence is a significant cause of physical and psychological harm and suffering for women and children. Although sexual violence mostly affects women and girls, boys are also subject to child sexual abuse. Nurse is the person who attends the victim first. ⋯ The WHO and IAFN have urged inclusion of forensic content in both undergraduate and postgraduate nursing programs. Forensic Nurse Specialist can provide direct services to individual clients, consultation services to nursing, medical and law-related agencies, as well as providing expert court testimony in areas dealing with trauma and/or questioned death investigative processes, adequacy of services delivered, and specialized diagnoses of specific medical conditions. Research Findings on the Effectiveness of Sexual Assault Nurse Examiner (SANE) Programs suggests various improvements in each and every step in care of victim of sexual assault.

The occurrence of a bad outcome, injury or death of a patient during treatment increases the chance of malpractice litigation, increases legal responsibility and leads to increased fees for malpractice insurance. Physicians practicing obstetrics and gynecology face among the highest risks of malpractice litigation, and such litigation has led to an increase in the practice of defensive medicine and has made this specialty less appealing. Previous clinical data from Saudi Arabia have shown that more malpractice litigation concerns claims in obstetrics and gynecology than claims in any other field of medicine. ⋯ Obstetricians face a high risk of malpractice claims in Saudi Arabia, although most claims do not end in payments to plaintiffs. However, the effects of such claims on obstetric care should not be underestimated. Adherence to standards of care and careful documentation may decrease litigation and the number of indefensible malpractice claims.
