Therapeutic approach to ovarian cysts in tamoxifen-treated women with breast cancer.
To present our clinical experience with gonadotropin-releasing hormone agonist (GnRHa) treatment of tamoxifen-treated women who had developed ovarian cysts, and to discuss the diagnostic and therapeutic options in such cases. ⋯ The findings of this preliminary report suggest that GnRHa might be an appropriate treatment for tamoxifen-treated women who develop ovarian cysts. It was demonstrated that the GnRHa caused regression of the ovarian cysts and enabled continuation with the adjuvant tamoxifen treatment.

To review cases of postpartum hysterectomy regarding indications, risk factors and complications and compare them with cases of emergency cesarean section. ⋯ Emergency postpartum hysterectomy is associated with significant blood loss, need for transfusion, postoperative complications and longer hospitalization partly because of its indications. The combination of prior cesarean section and current placenta previa should alert the obstetrician that an emergency postpartum hysterectomy may be needed.

Pulse oximetry is widely used to monitor the patient's well-being in anesthetic and neonatal practice. As a result of recent technologic and theoretical advances, it has emerged as a clinical tool in intrapartum fetal monitoring. Oximeters record both pulse rate and arterial oxygen saturation of the fetus and they may be adapted to derive an estimate of peripheral perfusion. ⋯ Fetal pulse oximetry is limited by a wide normal range and inadequate calibration. The amniochorionic membranes however do not affect oximetry readings so that this method may be applied before rupture of the membranes, i.e. before labor. Once successfully developed, fetal pulse oximetry could potentially be used in combination with other monitoring techniques to reduce instrumental and operative interventions during labor and improve perinatal outcome.
