In order to perform an automatic lamellar keratotomy microkeratomes are used, which differ from each other in several technical details. This study was done to examine, whether there are characteristics of the cutting edge, typical of each device, and whether there are some correlations between the cutting quality and technical parameters. ⋯ The quality of the cutting edge may be influenced by the relationship of the speed of the pass and the rate of blade oscillation/rotation. Therefore it seems that a lower feed during one oscillation/rotation results in a more smooth pattern of the cutting edge.

Visual disturbances after high altitude exposure were first reported in 1969. Later, the term "High Altitude Retinal Hemorrhage-HARH" has been used for the ensuing retinal hemorrhages and vascular engorgement. ⋯ The hypoxia at high altitude causes increased retinal blood flow and blood volume possibly via autoregulatory mechanisms. Furthermore, retinal venous pressure can be increased by extreme physical exertion and Valsalva maneuvers during mountain climbing. A hypoxic retinal capillary bed exposed to increased retinal venous pressure predisposes to intraretinal hemorrhage. Retinal changes include marked increase of retinal vessel diameter with tortuosity of arterioles and venoles and hyperemia or edema of the optic disc. The intra- or preretinal hemorrhages often spare the macular area. These patients do not experience debilitating symptoms unless vitreous hemorrhage occurs. This may be potentially hazardous when the patient is still in the high mountains. Clinically, all these retinal changes are reversible within weeks. To prevent high altitude retinopathy, ascending slowly and use of supplemental oxygen is recommended.

Case Reports[Endophthalmitis after intra-oral block of the infraorbital nerve].
Most penetrating needle puncture injuries occur in retro- or peribulbar anesthesia. Hereby only a small percentage of patients develop endophthalmitis. Ocular penetration after enoral infraorbital nerve block has not yet been reported in literature. ⋯ Careful anamnesis would have prevented this accidental globe penetration. Right upper palate is absent presumably due to congenital cleft malformation or surgery. This allowed needle penetration through smooth tissue into the right globe. Fortunately, endophthalmitis develops only in a small percentage after needle puncture. We recommend immediate pars-plana-vitrectomy and intravitreal antibiotics in case of endophthalmitis after ocular penetration.

Comparative Study[Clinical discrepancy between Schiötz and Perkins Tonometry in juvenile glaucoma].
In infantile glaucoma Schiotz and Perkins tonometry are often divergent. However, empirical data about the quantitative relation of Schiotz' and Perkins' measurements are lacking for this group of patients. ⋯ Marked discrepancy of applanation and indentation tonometry in infantile glaucoma is frequent. Decision for surgery should not base on tonometry alone, but on evaluation of additional clinical criteria like optic disc cup, corneal morphology and individual anamnesis.
