Randomized Controlled TrialRESCUE-AF in Patients Undergoing Atrial Fibrillation Ablation: The RESCUE-AF Trial
Although contact force (CF)–sensing catheters improve procedural effectiveness and safety of atrial fibrillation ablation, recent reports documented a higher incidence of atrioesophageal fistula formation relative to ablation with non–CF-sensing catheters.The present study was to assess whether restricting CF to <20 g reduced risk for esophageal injury (EI) in patients with atrial fibrillation undergoing circumferential pulmonary vein isolation. ⋯ Risk for EI was minimized when CF was restricted to <20 g at the posterior left atrial wall, where the circumferential pulmonary vein isolation lesion set and the course of the esophagus overlapped in all subjects.

Multicenter StudyEvaluating Real-World Clinical Outcomes in Atrial Fibrillation Patients Receiving the WATCHMAN Left Atrial Appendage Closure Technology: Final 2-Year Outcome Data of the EWOLUTION Trial Focusing on History of Stroke and Hemorrhage.
Left atrial appendage occlusion with WATCHMAN has emerged as viable alternative to vitamin K antagonists in randomized controlled trials. Evaluating real-life clinical outcomes in atrial fibrillation patients receiving the WATCHMAN left atrial appendage closure technology was designed to collect prospective multicenter outcomes of thromboembolic events, bleeding, and mortality for patients implanted with a WATCHMAN in routine daily practice. ⋯ URL: https://clinicaltrials.gov . Comparative StudyVentricular Fibrillation Waveform Analysis During Chest Compressions to Predict Survival From Cardiac Arrest.
Quantitative measures of the ventricular fibrillation (VF) ECG waveform can assess myocardial physiology and predict cardiac arrest outcomes, making these measures a candidate to help guide resuscitation. Chest compressions are typically paused for waveform measure calculation because compressions cause ECG artifact. However, such pauses contradict resuscitation guideline recommendations to minimize cardiopulmonary resuscitation interruptions. We evaluated a comprehensive group of VF measures with and without ongoing compressions to determine their performance under both conditions for predicting functionally-intact survival, the study's primary outcome. ⋯ VF waveform measures predict functionally-intact survival when calculated during chest compressions, but prognostic performance is generally reduced compared with compression-free analysis. However, support vector machine models exhibited similar performance with and without compressions while also achieving the highest area under the receiver operating characteristic curve. Such machine learning models may, therefore, offer means to guide resuscitation during uninterrupted cardiopulmonary resuscitation.

High-Resolution Infrared Thermal Imaging of the Esophagus During Atrial Fibrillation Ablation as a Predictor of Endoscopically Detected Thermal Lesions: Results From the HEAT-AF Study.
Endoscopically detected thermal esophageal lesions (EDEL) after ablation of atrial fibrillation may be precursors of atrioesophageal fistula and esophageal luminal temperature monitoring has previously failed to decrease thermal damage. ⋯ For the first time esophageal temperature monitoring using a high resolution, high-fidelity IRTS allowed accurate prediction of postablation EDEL suggesting that Tpeak alone is an excellent binary classifier of patients at risk of EDEL. The logistic regression model and associated receiver operator curve will aid in the selection of optimal temperature thresholds in future prospective studies.
