The purpose of this study was to investigate the ability of stroke volume variation (SVV) to predict fluid responsiveness in patients undergoing pulmonary lobectomy with one lung ventilation (OLV). Thirty patients intubated with double-lumen tube were scheduled for a pulmonary lobectomy requiring OLV for at least 1 hour under general anesthesia. Hemodynamic variables including heart rate, mean arterial pressure, cardiac index (CI), stroke volume index (SVI), central venous pressure (CVP) and SVV were measured before and after volume expansion (VE) (8 mL/kg of 6% hydroxyethyl starch). ⋯ The baseline value of SVV, CVP, CI and SVI did not correlate significantly with ΔCI (p > 0.05). The area under the Receiver Operating Characteristic (ROC) curve were 0.507 for SVV (95% confidence interval, 0.294-0.720) and 0.556 for CVP (95% confidence interval, 0.339-0.773), neither was able to predict fluid responsiveness with sufficient statistical power. SVV measured by the Vigileo-FloTrac system was not able to predict fluid responsiveness in patients undergoing pulmonary lobectomy with OLV after thoractomy.

Idiopathic pulmonary fibrosis is an intractable disease with a median survival time of 2 to 3 years. Serum levels of Krebs von den Lungen-6 (KL-6), surfactant protein A (SP-A), and surfactant protein D (SP-D) are useful biomarkers for idiopathic pulmonary fibrosis and they are widely used in Japan. ⋯ However, the differential diagnosis of idiopathic pulmonary fibrosis from other interstitial lung diseases is still challenging. Several other biomarkers have been identified and are being studied in Japan.

The hyperglycemic response is an important prognostic factor for survival after hemorrhage. In this study, we investigated the effects of glucose administration during volume resuscitation from hemmorhagic shock in fasted rats under sevoflurane anesthesia on hemodynamics, acid/base-balance and glucose metabolism. Hemorrhagic shock was induced in rats by withdrawing 25 mL/kg of blood. ⋯ After volume resuscitation, group-Dextran[glucose] showed significantly higher glucose levels, significantly higher insulin levels and significantly lower lactate levels in comparison with group-Dextran[saline]. At 30 min after volume resuscitation, base excess correlated significantly with lactate levels. These results suggest that glucose administration during volume resuscitation using dextran-40 from hemorrhagic shock ameliorates acid/base-imbalance associated with hyperlactatemia in fasted rats under sevoflurane anesthesia.

Respiration variation in arterial pulse pressure (ΔPP) and pulse oximetry plethysmographic waveform amplitude (ΔPOP) are accurate predictors of fluid responsiveness in mechanically ventilated patients. We hypothesized that stroke volume variation (SVV) and pleth variability index (PVI) can predict fluid responsiveness in mechanically ventilated patients during major surgical procedures in Hans Chinese. This prospective study consisted of fifty-five Hans Chinese patients undergoing resection of primary retroperitoneal tumors (PRPT). ⋯ The best threshold values to predict fluid responsiveness were more than 12.5% for SVV and more than 13.5% for PVI in the real surgical setting. The baseline value of SVV, and PVI correlated significantly with volume-induced changes in SVI (p < 0.01). Both SVV and PVI could be used to predict intraoperative fluid responsiveness during resection of PRPT in Hans Chinese.

Information about medical students' choice of specialty can be helpful for planning health manpower. However, such information from medical students in Malaysian medical schools is lacking. We carried out a cross-sectional questionnaire survey among fourth- and fifth-year medical undergraduate students at Melaka-Manipal Medical College. ⋯ About half of the students intended to pursue their postgraduate studies in Malaysia, most of the rest in the United Kingdom or Australia. While internal medicine and surgical subspecialties were preferred, students were not inclined towards primary care or diagnostic subspecialties. Incentives should be provided and other measures should be taken to make these branches more attractive.
