Multiple joint involvement in total knee replacement for osteoarthritis: Effects on patient-reported outcomes.
To determine whether symptomatic (painful/problematic) joints pre-total knee replacement (TKR) surgery influence 1) pre- and 12-month post-TKR patient-reported outcomes (pain, physical function, and mood [fatigue, anxiety, and depression]) and 2) postsurgical pain and function mediated through mood. ⋯ Findings suggest that a comprehensive approach to osteoarthritis management/care is warranted, and identify important associations between symptomatic joints and mood that negatively impact post-TKR pain and physical function.

Potential impact on patient residence to hospital travel distance and access to care under a policy of preferential referral to high-volume knee replacement hospitals.
To examine the potential impact of a policy of selective referral to high-volume knee replacement hospitals on patients' travel distance to hospitals and access to care for patients seeking total knee replacement (TKR) in urban and rural settings. ⋯ A policy to direct patients away from LVHs could increase patients' travel time to hospitals in rural areas and restrict access for minority and low-income patients in urban areas. Any implementation of selective referral to high-volume centers should address access to hospitals for rural patients and urban minority and low-income patients.

Comparative StudyPredictors of health-related quality of life in children and adolescents with juvenile idiopathic arthritis: results from a Web-based survey.
Children with juvenile idiopathic arthritis (JIA) experience functional impairment due to joint manifestations of the disease. The aim of our present study was to assess health-related quality of life (HRQOL) and its predictors in a group of children and adolescents with JIA. ⋯ The HRQOL is severely affected in children and adolescents with JIA. These findings underline the necessity to systematically monitor HRQOL in daily clinical practice.

Comparative StudyHip fractures in the United States: 2008 nationwide emergency department sample.
To evaluate the recent epidemiology of hip fractures in the US. ⋯ The slowed growth of hip fracture risk after age 85 years suggests that the eldest old group may have a distinct hip fracture risk. Our study showed that trochanteric fractures were twice as common as cervical fractures. Because trochanteric fractures are more closely related to severe and generalized bone loss than cervical fractures, we hypothesize that the high incidence rate of trochanteric fractures in the US suggests that osteoporosis is a health problem that is linked to hip fracture. In addition to improved safety measures to reduce falls, rigorous preventive treatments of osteoporosis may be needed.

Review Practice GuidelineAmerican College of Rheumatology 2012 recommendations for the use of nonpharmacologic and pharmacologic therapies in osteoarthritis of the hand, hip, and knee.
To update the American College of Rheumatology (ACR) 2000 recommendations for hip and knee osteoarthritis (OA) and develop new recommendations for hand OA. ⋯ These recommendations are based on the consensus judgment of clinical experts from a wide range of disciplines, informed by available evidence, balancing the benefits and harms of both nonpharmacologic and pharmacologic modalities, and incorporating their preferences and values. It is hoped that these recommendations will be utilized by health care providers involved in the management of patients with OA.
