Surgical treatment of extra-articular distal-third diaphyseal humeral fractures is controversial in terms of surgical approach and position of implant. The aim of this study is to evaluate the clinical and radiological outcomes of a modified application of the proximal humeral internal locking system (PHILOS) plate in extra-articular distal-third diaphyseal humeral fractures. ⋯ Therapeutic level IV, case series.

Metabolic syndrome has been known as a risk factor for morbidity following orthopedic procedures, yet its impact on surgical treatment of ankle fractures remains unclear. The aim of this study was to compare the patient outcomes of surgical treatment of supination-external rotation ankle fractures in patients with and without metabolic syndrome. This study was designed as a retrospective matched case-control study. ⋯ The K&L scale and complications did not differ significantly between the two groups. Patients with metabolic syndrome are at risk for higher pain scores and lower functional outcomes after surgical treatment for supination-external rotation ankle fracture. These results suggest that metabolic syndrome should be treated together with ankle fractures.

We sought to determine the effects of sociodemographic factors on the occurrence of orthopaedic injuries in an adult population presenting to a level 1 trauma center. ⋯ The results from this study indicate that a link exists between sociodemographic factors and the occurrence of orthopaedic injuries presenting to a level 1 trauma center. The most common cause of injury varied within age groups, by sex, and within the different socioeconomic groups.

Humeral shaft is a common site of fracture non-union. Biology and bone quality represent some of the problems that the orthopaedic surgeon has to face up in the elderly. The goals of treatment of humeral shaft non-union are the achievement of mechanical stability and creation of a favourable biologic environment. Bone graft and stem cells are some of the augmentation techniques available to reach these goals. ⋯ The use cortical allograft with stem cells is a viable strategy to treat humeral shaft non-union in the elderly.

For trochanteric fractures, helical blade placement is crucial to the prognosis of operations. Existing measurement methods used for blade placement include the Cleveland zone, the tip-apex distance (TAD), the calcar-referenced tip-apex distance (CalTAD), and the Parker's ratio. These methods all lack a direct view on blade direction. The current study proposed the axis-blade angle (ABA) to solve direction problem and investigated its clinical applicability. ⋯ The ABA provides instruction for the intraoperative adjustment of guide wire direction. Placing the helical blade with an ABA > -10° can effectively reduce the risk of mechanical complications.
