[Role of university hospitals in Bavaria during the COVID-19 pandemic].
In early 2020 the German healthcare system was put into a state of emergency due to the coronavirus disease 2019 (COVID-19) pandemic. Bavaria had to deal with more severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infections than any other German state during the first wave and currently has over 270,000 cases, accounting for about one fifth of all COVID-19 cases in Germany. The Bavarian Ministry of Interior together with the Bavarian Sate Ministry of Health and Care issued a general ruling at the beginning of the first wave that ordered the centralised organisation of hospital capacity, a redesign of the information technology (IT) management system and introduced reporting obligations for SARS-CoV-2/COVID-19. The goal of this analysis was to investigate the role that university hospitals played in the inpatient treatment of COVID-19 patients. ⋯ During the first wave university hospitals provided relevant scientific contributions and played an important role in advising physicians, hospitals and politicians on the pandemic. In all, 20% of intensive care unit (ICU) and intermediate care (IMC) treatment days were provided by the university hospitals in particular for treatment of complex courses of COVID-19.

Review[Core curriculum Medical intensive care medicine of the German Society of Medical Intensive Care and Emergency Medicine (DGIIN)].
Medical intensive care medicine treats patients with severe, potentially life-threatening diseases covering the complete spectrum of internal medicine. The qualification in medical intensive care medicine requires a broad spectrum of knowledge and skills in medical intensive care medicine, but also in the general field of internal medicine. ⋯ This curriculum is primarily dedicated to the internist in advanced training in medical intensive care medicine. However, this curriculum also intends to reach trainers in intensive care medicine and also the German physician chambers with their examiners, showing them which knowledge, skills as well as behavior and attitudes should be taught to trainees according to the education criteria of the German Society of Medical Intensive Care and Emergency Medicine (DGIIN).

[Risk stratification through implementation and evaluation of a COVID-19 score : A retrospective diagnostic study].
The clinical manifestation of COVID-19 is nonspecific and varies greatly, which makes it more difficult to discriminate from other (virus) infections. Neither individual findings nor combinations of findings are specific enough to be able to diagnose COVID-19 with a high degree of certainty. The goal was to identify patients in the emergency department, who are at risk for COVID-19 disease, early by using a score, so that they could be isolated pre-emptively. ⋯ The score, which is easy to obtain during the initial assessment, supports the assessment of the pretest probability for a COVID-19 infection as part of the risk stratification and can influence the treatment pathway in terms of pre-emptive isolation, PCR testing and other treatment options at an early stage. Due to the nonspecific symptoms of the disease; however, it must be accepted that the goal of high sensitivity results in a relatively low specificity of the score.

ReviewGerman recommendations for critically ill patients with COVID‑19.
Since December 2019, a novel coronavirus (severe acute respiratory syndrome-coronavirus 2, SARS-CoV-2) has rapidly spread around the world resulting in an acute respiratory illness pandemic. The majority of patients presents with mild symptoms of coronavirus disease 2019 (COVID-19). However, about 5% become critically ill and require intensive care treatment. ⋯ Early intubation and repeated prone positioning are key elements in treating hypoxemic COVID-19 patients. Strict adherence to basic infection control measures (including hand hygiene) and use of personal protection equipment (PPE) are essential in the care of patients. Procedures that lead to formation of aerosols should be avoided where possible and carried out with utmost precaution.
