Conversions to open surgery during laparoscopic cholecystectomy are performed in 20% of patients with acute cholecystitis, and are associated with increased morbidity and costs. The aim of this study was to identify predictive factors for conversion and to evaluate morbidity, mortality and hospital stay. ⋯ It is important to recognise patients with a higher risk of conversion in order to optimise planning and performing of the surgical procedure, and to decrease the morbidity associated with laparotomy, given that the independent factors identified are not modifiable.

Despite there being no evidence of the advantages of its use, mechanical bowel preparation (MBP) continues to be routine in colorectal surgery. Our objective is to analyse the impact of its selective use, as regards patient comfort and results, comparing a perioperative multimodal rehabilitation program (MMRH) with conventional care (CC). ⋯ The restriction of MBP is safe, and associated with an MMRH program, contributes to a faster and more comfortable recovery, without increasing complications.

[Anaemia, iron, transfusion and therapeutic alternatives. A review from a surgical perspective].
Anemia is very common entity or comorbidity in surgical patients. Its management involves a multidisciplinary approach with the aim of optimizing the available therapeutic resources with individualized care for each clinical situation. Rational use of blood transfusions, iron therapy (oral and intravenous), erythropoiesis stimulating agents and other therapeutic alternatives by physicians must achieve maximal benefit with minimal complications for our patients. This review article summarizes the main characteristics of anemia, iron metabolism, erythropoiesis and therapeutic alternatives from a surgical perspective in the light of present knowledge.
