Randomized Controlled Trial[Diclofenac during prostate needle biopsy. Significant pain reduction].
The aim of this study was to evaluate the efficacy of diclofenac to reduce pain during prostate biopsy. ⋯ The preinterventional administration of diclofenac suppositories is a simple but efficient procedure for pain reduction in patients who undergo prostate biopsy.

Pathomorphological findings for the male genital are an exception among forensic cases. In earlier publications they were usually published as individual cases. A significant percentage are the so-called fertility- and potency medical certificates, which are usually commissioned for doubtful paternity cases. ⋯ Another group consists of reports about self-inflicted genital mutilation, e.g. among prison inmates or soldiers of the last world wars to become an invalid. When it was necessary to identify a corpse, e.g. in cases of progressive decomposition, Merkel recommended in 1945 the representation of the so-called "Schnepfenkopf" (snipe head; colliculus seminalis) in the lesser pelvis. In this article a medical and forensic review of forensically relevant findings of the male genital is presented and illustrated with historical cases.

Case Reports[Inappropriate use of a titanium penile ring. An interdisciplinary challenge for urologists, jewelers, and locksmiths].
Constricting devices placed on the penis to improve sexual performance present a challenge to urologists. Various metallic and nonmetallic objects have been described -- plumbing cuffs, bull rings, wedding rings, plastic or rubber rings -- which usually could be removed by the combination of medical and operative treatment (decompression of the penis and glandular puncture) and craftsmen's techniques. We describe the case of a titanium thumb ring of 2 mm thickness which only could be removed by using an electric cutting tool.

Review[Fast-track surgery in radical retropubic prostatectomy. First experiences with a comprehensive program to enhance postoperative convalescence].
Fast-track surgery is a comprehensive program for the optimization of perioperative care in elective surgery reducing potential postoperative complications and speeding up convalescence. Recent data from randomized colon resection trials emphasize that fast-track surgery is possible in most major operations. Our initial results in radical retropubic prostatectomy fast-track surgery have been encouraging. Fast-track surgery in major urological operations needs validation using randomized trials.

In the last 10 years in Germany we have seen a lot of hernia repairs using mesh. Meta-analysis shows the advantages of using meshes in hernia surgery; recurrence rates in inguinal hernia surgery are less than 3% in studies. ⋯ In incisional hernia surgery there is no discussion about using meshes. The role of minimally invasive surgery has not yet been defined.
