Video-fiberoscopy is the exploration of upper respiratory airways used in our diagnostic protocol in patients with chronic snoring and obstructive sleep apnea syndrome (OSAS). We perform a fibroscopy during propofol induced sleep without tracheal intubation; video recording and monitoring of level of sleep, pulse oximetry, blood pressure, ECG, etc. is used. This exploration allows us to asses upper respiratory airways when the patient is asleep as well as the areas of collepse and vibration both from a functional and anatomical points of view.

Case Reports[Subdural empyema of sinus origin caused by Gemella morbillorum, a strange etiology].
We present a case of frontal sinusitis complicated with a subdural empyema, in which the identified microorganism was Gemella morbillorum, a frequent host of the aerodigestive tract and occasionally related to infections. The problem was resolved successfully using endoscopic surgery and an external approach of the sinus. ⋯ Subdural empyema is a rare complication of sinusitis although very severe. We want to emphasize the importance of early diagnosis of intracranial complications, the need of a detailed microbiology test the method used to obtain samples, and the convenience of a combined approach by the otolaryngologists and the neurosurgeons for its complete drainage.

[Treating benign paroxysmal positional vertigo with the canalith repositioning maneuver of Epley. Our experience].
Benign Paroxysmal Positional Vertigo (BPPV) is one of the most common peripheral vestibular disorders. Particle repositioning maneuvers are simple and effective treatments for patients with BPPV. This study included 37 consecutive cases of BPPV treated with the particle repositioning maneuver of Epley. 97% of patients improved and the maneuver was well tolerated. We conclude that Epley Maneuver is safe and effective in the treatment of BPPV.

Myasthenia gravis is an illness of insidious onset whose first manifestation often is dysphagia. We report the case of a 25 year-old woman who consulted for nosebleed. On her first visit she referred progressive dysphagia, but the results of endoscopic digestive examination and indirect laryngoscopy were normal. ⋯ Myasthenia gravis was diagnosed by means of electromyography and determinations of antibodies against acetylcholine receptors. We reviewed the literature on the clinical manifestations, diagnostic procedures, and treatment of this condition. This case highlights the importance of suspecting myasthenia gravis in young women with dysphagia and a family or personal history of autoimmune disease in order to reach an early diagnosis and treatment.

Case Reports[Synchronous osteochondroid hamartoma and squamous cell carcinoma of the larynx].
Soft-tissue tumors of the larynx are rare, especially hamartomas. Fewer than a dozen well-documented cases have been described. We report the case of a 65 year-old man diagnosed of a synchronous epithelial tumor of the right vocal fold and a soft-tissue tumor of the contralateral lamina of the thyroid cartilage. ⋯ To our knowledge, this is the first case of a synchronous laryngeal hamartoma and carcinoma reported in the literature.
