Lemierre's syndrome or postanginal septicaemia is described in the literature as an unusual complication of oropharyngeal infection. In most cases the syndrome is caused by Fusobacterium necrophorum. This disease usually occurs in young healthy individuals with an oropharyngeal infection, implying high fever, cervical tumefaction, and cervical pain, frequently associated with septic lung embolization. This case report is an unusual variant of this disease in a 12-year-old patient whose site of primary infection was the middle ear.

[Treatment of epistaxes in hereditary haemorrhagic telangiectasia (Rendu-Osler-Weber disease) with tranexamic acid].
Recurrent epistaxis is the most frequent clinical manifestation of hereditary haemorrhagic telangiectasia (HHT). Its treatment occasionally presents difficulties as there is no consensus on the appropriate therapeutic protocol. Our objective was to explore the utility of oral tranexamic acid for the treatment of epistaxes in HHT patients. ⋯ Oral tranexamic acid is useful for achieving significant reductions in epistaxis frequency and intensity in selected patients with HHT. In those presenting severe epistaxis, however, it may need to be combined with more aggressive therapies.

Case Reports[Fracture of thyroid cartilage after a sneezing episode].
We present a case report of a thyroid fracture after a sneezing episode, with odynophagia, dysphonia, and neck pain. The examination showed oedema at the right vocal cord and haematoma at the right false vocal cord. An anterior thyroid fracture without displacement, and a subcutaneous emphysema could be seen on the CT. Thyroid fracture because of this aetiology is most exceptional; only 1 similar case report has been described in the literature.

Case Reports[Migration and extrusion from the upper digestive tract to the skin of the neck of a foreign body (fish bone)].
Foreign bodies ingestion is a common problem seen at emergency rooms and mostly involved are fish and chicken bones. The diagnosis can be difficult because of the age of the patient (children and older patients with dental prosthesis). ⋯ No intervention was needed. A discussion of the management of migrated foreign bodies follows.

[Clinical tumoral size dissociation in acoustic neuroma: reality or measure distortion?].
In this study we have analyzed (i) the audiometric frequencies more often affected in acoustic neuroma (AN), (ii) the percentage of patients presenting normal hearing and those with sudden hearing loss, (iii) if there is a correlation between tumor size and hearing loss, and (iv) the relationship between clinical and radiological parameters and audiological data. ⋯ The association between hearing loss and Vth cranial nerve deficit, and between hearing loss and preoperative facial palsy is independent the size of the tumour.
