Comparison of bioimpedance and thermodilution methods for determining cardiac output: experimental and clinical studies.
The changes in electrical bioimpedance caused by the blood flow through a thoracic segment may be measured using a series of electrodes placed at opposing ends of this segment. Cardiac output (CO) is calculated by computer as the change in bioimpedance over time. This study was performed to determine the accuracy of bioimpedance CO (CObi) compared with standard thermodilution CO (COtd) in an animal model and in patients. ⋯ CObi became inaccurate with severe tachycardia (pulse, 180/min), low CO, or frequent arrhythmias. These results indicate that this technique provides a continuous noninvasive method of determining CO that is comparable with thermodilution techniques. The technique of CObi holds promise for widespread use when continuous and noninvasive monitoring of CO is necessary.

Aortic stenosis accounts for 5 to 6% of infants and children seen for surgical repair of congenital heart disease. The clinical presentation and reported results of operation for aortic stenosis are highly variable. This retrospective review was undertaken to assess our operative mortality and the degree of gradient reduction afforded by each of several surgical techniques used to treat aortic stenosis in children over a 30-year period. ⋯ Twenty-nine (13%) of the 223 long-term survivors have undergone a subsequent procedure for relief of residual or recurrent obstruction; 12 have had insertion of an aortic valve prosthesis, 12 have had insertion of an apicoaortic conduit, and 6 have required repeat aortic valvotomy. These data demonstrate the low operative mortality and excellent hemodynamic benefit of surgical relief of single-level aortic stenosis in children older than neonates. Conduits placed for complex obstructions or operative procedures in neonates have acceptable hemodynamic benefits, but operative mortality remains high.

Unilateral reexpansion pulmonary edema (RPE) is a rare complication of the treatment of lung collapse secondary to pneumothorax, pleural effusion, or atelectasis. Although RPE generally is believed to occur only when a chronically collapsed lung is rapidly reexpanded by evacuation of large amounts of air or fluid, in this review 15 of 47 cases of RPE available for assessment occurred when the pulmonary collapse was of short duration or when the lung was reexpanded without suction. ⋯ Implicated in the etiological process of RPE are chronicity of collapse, technique of reexpansion, increased pulmonary vascular permeability, airway obstruction, loss of surfactant, and pulmonary artery pressure changes. Since the outcome of RPE was fatal in 11 of 53 cases reviewed (20%), physicians treating lung collapse must be aware of the possible causes and endeavor to prevent the occurrence of this complication.

Comparative morbidity of mitral valve repair versus replacement for mitral regurgitation with and without coronary artery disease.
Mitral valve repair has been increasingly used at our hospital for mitral regurgitation with and without coronary disease. From January, 1984, to June, 1987, of 338 patients undergoing all forms of mitral valve surgery, 140 had first-time surgery for pure mitral regurgitation: 75 had valve repair, and 65 had valve replacement. Thirty-three of 75 (44%) had concomitant coronary bypass in the repair group, while 21 of 65 (32%) had coronary bypass in the replacement group. ⋯ There were 7 late deaths in the replacement group and only 3 late deaths in the repair group. Actuarial survival at 30 months was 85 +/- 6% for the replacement group and 94 +/- 4% for the repair group. There were 5 late emboli (1 fatal, 4 nonfatal) after valve replacement and none after valve repair (p = 0.03).(ABSTRACT TRUNCATED AT 250 WORDS)

Spinal cord injury following operations on the descending thoracic or thoracoabdominal aorta remains a major problem. In certain subsets of patients, the risk of postoperative spinal cord injury is substantial. Although several adjuncts have been employed clinically to eliminate or reduce the frequency of this complication, none have proven to be completely effective. ⋯ Since postoperative spinal cord injury most likely results from ischemia or hypoxia of the lower segment of spinal cord, use of adjunctive techniques to preserve spinal cord function during aortic clamping by perfusing the distal aorta adequately with or without systemic hypothermia should be considered. To practically implement this, partial cardiopulmonary bypass for distal perfusion when the critical intercostal or lumbar arteries originate from the aorta distal to the excluded segment, and total cardiopulmonary bypass with systemic hypothermia and implantation of intercostal and lumbar arteries when these arteries originate from the excluded segment, can be used. In addition, whenever possible, intraoperative monitoring of spinal cord function should be performed.
